On August 17, 2010, a roof fall was discovered at crosscut #28, Main West Section, #1 entry.
A piece of coal struck employee in ankle. Medical was sought, no fracture, told ankle would get better, full release. Ankle not getting better, appointment was scheduled with orthopedic DR., before appointment there was minor re-aggravation. Feb 10, 2011 he attended scheduled appointment, became LTA with soft tissue injury. Late reporting due to medical records gathering.
Employee was operating a cansetter, hit a small drop off causing the employee to strike his head against cab of machine. Doctor excused employee from work.
Employee alleged that he hurt his shoulder as he was pushing up on ventilation tubing that was hanging from the roof of the mine. On 7-21-2010, the employee received treatment for impingement syndrome and supraspinatus tendontis, with the impingement as a result of chronic bone spurs. This is being classified as an illness.
Injured worker and fellow worker were pulling up water hose in preparation to wash equipment on the longwall. As the hose was being pulled the valve caught causing the hose to turn on. The hose whipped up and struck employee. The employee received stitches, broken tooth and time away from work.
Employee was stepping from a vehicle, did not notice drop in floor level. As his feet engaged the floor, he felt pain in his back. He was released to work but the condition continued to worsen. Employee received surgery on May 10, 2010, causing this to be a lost time. Medical professionals had to advise whether this was an illness or injury before reporting.
While bolting ribs, employee twisted his back as he was installing a truss bolt. Medical attention was not sought until July 19, 2010 and at that time he was placed on restricted duty.
EE was working on Tub of a dragline. He was standing on the 3rd step of a 6' ladder. As he went to step down off the ladder, his foot slipped on the 2nd to bottom rung. When his foot slipped, he struck his chest on the Tub Manway Portal in The Tub.
Employee was spot bolting, as he was inserting the resin he felt pain in his back.
Employee slipped off of the lower step on a fuel truck and landed on the door threshold, injuring his right back rib area.
Employee was in the process of moving ventilation tubing, during this process a piece of cap rock fell and struck the employee in the head. Medical personnel has placed the employee on restricted duty as precautionary due to the possibility of a slight concussion.
The employee was helping install half of a hydraulic to the other half of the hydraulic tank by lifting one side of the tank so a fellow employee put bolts in the tank. While lifting the side of the tank the employee felt a pop in his lower back.
While welding some hot slag fell onto some water on the mine floor. The water caused the slag to pop back up behind employee's welding helmet and safety glasses. The employee received a prescription for his eye.
Employee was installing clips on a Kennedy stopping, he was using a ladder in performing this task. The ladder settle in the mud, employee grasp a panel, cutting his finger. The injury required stitches to close.
Employee had been taking coal samples and placing them in 55 gallon barrels. Next morning he noticed a bulge in his abdomen. Later, employee sought medical attention and was diagnosed with a hernia. Hernia has been repaired.
Employee was loading a trailer with supplies, dust rolled back under safety glasses into miners eye.
Employee was in process of repairing long wall shield, he accidentally turned on a valve supplying pressurized emulsion to a hose that he was going to hook up to the shield. The pressurized hose struck employee in the head and also put emulsion into his eye. He received prescription medication for his eye.
While attempting to free up the gearing of a hoist with the use of a screw driver, the screw driver slipped allowing employees finger to come in contact with one of the gears of the hoist. Employees finger was cut, requiring stitches.
