Employee requested medical treatment on July 9th and was placed on restricted duty. Employee stated he was turning water valve using a wrench. During the process, the wrench was stuck and when he applied additional pressure, the wrench slipped off the valve and alleged he fell backwards. Diagnosed with lumbosacral sprain, lower and upper back contusion(s), and back abrasion.

The employee was treated at the company clinic on 5/22/12 and was placed on restricted duty. While exiting the water truck, the employee failed to maintain 3 points of contact. This resulted in a fall and a fracture to her left wrist.
On 04/10/2012 employee was taken to the company clinic, was diagnosed with a right shoulder sprain and was placed on restricted duty. The employee reported while descending the ladder on a loader, his right foot slipped off causing him to lose grip with left hand and placed weight on right arm and shoulder.
The employee received dental treatment on 8/29/12, beyond first aid. The employee whipped a chain and hit his face resulting in a broken tooth.
On 6/1/12 the employee requested medical treatment, was taken to the company clinic and was prescribed physical therapy for a sprained left shoulder. The employee alleged that he jammed his L-shoulder on an arm rest of a loader while stock piling. He was returned to full duty on 6/1/12.
