An employee was helping another mag drill holes. The plate was cleaned and drill clear, another employee turned it on at the same time IE reached back in to move a rag. It became entangled in the bit and around IE's glove. This caused lacerations to ring finger and lacerations/fractures to the tip of middle finger, resulting in the loss of 1/2 inch of the tip of middle finger.

Employee exited the excavator EE was operating. Upon exiting EE stepped onto an unstable piece of concrete which caused EE to lose balance, twisting ankle and falling over.
A water pump in the pond was being replaced by 6 employees. One employee was holding a 48"" pipe wrench holding the shaft in place. Another employee was using a 36"" pipe wrench to turn the shaft to break it free. As the employee turned the shaft, the employee holding the pipe wrench lost grip and the tool swung around coming off striking an employee in the head above left ear.
EE was removing the segments on 988G loader bucket. EE was using 3/4"" air impact gun to remove nuts from plow bolts. The bolt loosened, descended allowing it to spin. EE used palm to push the bolt back to lock it in place to allow impact to remove the nut from top. The plow bolt engaged EE's glove pulling EE's hand into it and caused a severe cut across the right palm/thumb.
Employee alleges that while shoveling material onto the conveyor, EE experienced a strain to EE's right shoulder. On October 1, 2021, the employee requested medical treatment; EE was treated at the clinic where EE was prescribed work restrictions.
Employee exited water wagon which was parked by water stand to turn off water valve which is located 20' from water wagon. Employee stated they stepped on soft spot and twisted ankle.
While performing daily clean up of the sweeper truck, employee experienced slight discomfort in right shoulder while removing the suction hose.
Employee was operating the water truck when EE began experiencing gradually increasing back pain. On 4/12/21 EE requested medical treatment, and was prescribed physical therapy.
While training another person, employee looked back at the drill rig, the operator motioned the drill pipe. Employee noticed the drill pipe fell out of gripper arm. When employee touched the pipe to assist it back into the gripper arm. The pipe dropped and pinched the tips of fingers 3 & 4 on right hand
During the installation of rebar for the surge tunnel, a piece of #10 rebar was being placed and dropped. The bar was being installed by several men on a wall. The bar that fell was about 51'-0 long. It was being tied horizontally. When they were handing the bar across it slipped out and fell, landing on EE's wrist.
EE was welding in the primary feeder, while welding EE suffered flash burn not knowing until the next day. Cause was possible flash reflecting off the feeder coming through the back of EE's welding hood.
Employee was attempting to reposition conveyor belt back on track and heard a pop in EE's shoulder. On 1/21/21 EE was diagnosed with a right shoulder strain and was prescribed physical therapy.
