Struck against a moving object

EE was operating CAT D25C articulated truck & was backing up to get loaded by the track hoe. EE was looking over his right shoulder to look up at the track hoe. EE stated that the weight of his hard hat & the positioning of his head caused him to strain his neck. He said that his truck was moving slow & the ground was rough. EE did not report neck strain until 2:30 pm.

Struck against a moving object

Co-worker was in a CAT 637E Scraper and recklessly hit the back end of EE's CAT 631 Scraper.

Fall from machine

EMPLOYEE CLIMBED ONTO THE DOZER ON TRACKS, LOST BALANCE AND FELL OFF DOZER.

Caught in, under or between a moving and a stationary object

Employee was taken to Kaiser Permanente on 7/17/04 and was taken off work. High-pressure water hose was tangled in the head pulley and gravity take-up snubber pulley, causing it to pull employee's right arm between idler and the moving conveyor belt resulting in two compound fracturers.

Over-exertion in lifting objects

Employee was replacing paddle hubs in blade mill. While lifting and moving hub on shaft he allegedly strained back. Employee went home at end of shift. He was transported from his home to the E.R. on the morning of December 16th, complaining of severe back pain. He was diagnosed with a strained back muscle.

Fall from machine

Employee went to see company doctor on 11/4/2004 and was placed on restricted duty. Employee was climbing up the side of the jaw crusher to look inside, he slipped off the step and fell to the catwalk below, which resulted in a contusion to the left ribs and left arm.

Struck by falling object

Employee went to see company doctor on 10/27/04 and was placed on modified duty. When putting together shelf units, the top shelf fell and fractured employee's right thumb.

Employee went to see company doctor on 9/30/04 and was placed on restricted duty. Employee was attempting to remove a wedged rock from inside of loader frame by striking it with his right foot which caused a strain to his right calf.

Employee went to see company doctor on 9/30/04 and was placed on restricted duty. Employee, while sliding himself on concrete floor under loader to install front differential, punctured his left hand/arm.

Employee went to see company doctor on 8/21/04 and was placed on restricted duty. Employee was placing various tools in a pile, wrench, screwdrivers, etc. and strained his lower back.

Employee went to see company doctor on 8/17/04 and was placed on restricted duty. Employee opened hood of yard truck for a pre-trip inspection prior to operating vehicle. Hood went past normal stopping point because safety catch was broken; employee was attempting to hold hood with his left arm straining lower back and rib cage.

Employee went to see company doctor on 7/28/04 and was placed on restricted duty. Employee was helping electrician fill trench with concrete. Smashed finger when flipped chute down, causing contusion and open wound to his right index finger.

Employee went to see Company doctor on 06/14/2004 and was released on restricted duty. Employee was climbing up ladder of loader and slipped twisting his ankle and fracturing his R-heel.

Employee went to see company doctor on 5/10/2004 and was placed on restricted duty. Employee was performing service on dozer when he slipped due to grease/oil on sole of shoes and strained his back.

EE WENT TO SEE COMPANY DR. ON 3/19/04, STITCHES WERE APPLIED AND HE WAS PLACED ON RESTRICTED DUTY. WHILE REMOVING NUTS AND BOLTS FROM A LOADER BUCKET, EE CUT HIS LEFT INDEX FINGER CAUSING A LACERATION.

Employee went to see company doctor on 3/11/04, received stitches and was placed on restricted duty. Adjusting forks on forklift to pick up palet. Fork fell against back brake causing an avulsion and laceration to two left fingers.

Employee went to see company doctor on 3/12/04 and was placed on restricted duty. Employee stepped onto conveyor for a mechanical repair causing internal derangement to left knee.

EE WENT TO SEE COMPANY DOCTOR ON 1-24-04 AND WAS PLACED ON RESTRICTED DUTY. EE SLIPPED OFF STEP WHILE ASCENDING CRANE WHICH CAUSED AN ABRASION AND CELLULITIS TO HIS LEFT LEG.

EMPLOYEE WENT TO SEE A DOCTOR ON 1/16/04 AND WAS PLACED ON RESTRICTED DUTY UNTIL 1/19/04 DUE TO A FRACTURE TO HIS TOE. EMPLOYEE WAS TURNING OVER A SKID STEER BUCKET TO INSTALL A NEW BOTTOM PIE CE AND BUCKET DROPPED ON HIS LEFT FOOT CAUSING A CONTUSION TO LEFT FOOT AND A FRACTION TO HIS LEFT TOE.

Employee was using hammer on clamp rail of second screen deck. Hammer struck upper deck frame glancing off course and striking employee on left thumb.

Employee went to see company doctor on 5/26/04. Stitches were applied and employee was returned to full duty. Employee was removing/installing teeth on loader when a small piece of steel broke and became imbedded in his R-leg.

Employee went to see company doctor on 4/3/04 and prescription medication was issued for right eye. While cleaning eccentric tube of shaker, particle of sand, from unknown source, entered right eye which caused an abrasion.

WHILE RUNNING DRAGLINE DRAGGING BANK, THE CABLE BROKE AND FLEW THROUGH THE OPEN WINDOW STRIKING EMPLOYEE'S LEFT ELBOW AND BOTH HANDS CAUSING LACERATIONS AND A CONTUSION. EMPLOYEE WENT TO SEE C OMPANY DOCTOR, STITCHES WERE APPLIED AND HE WAS PLACED ON RESTRICTED DUTY. (EMPLOYEE QUIT THE NEXT DAY).
