Struck against a moving object

On 11-14-13 the employee requested medical treatment, was taken to the Company clinic and was placed on restricted duty which is being accommodated. The employee alleged that operating the excavator and possibly the bobcat caused pain in his lower back.

Over-exertion in lifting objects

On 5/6/13 the employee requested med treatment, was taken to the Company clinic & was placed on restricted duty which was accommodated. The employee alleged that while lifting & pushing a water hose into the dust suppression tank, he felt pain in his low back. It is unknown if this alleged event occurred on mine property & it is currently being investigated by the WC TPA.

Struck by... (Not Elsewhere Classified)

On 11/6/13 the employee was taken to the hospital and received stitches for a laceration. While grinding on a crusher rotor, the grinder kicked back and struck the employee in the mouth resulting in a laceration to the upper lip. The employee was released to full duty on 11/7/13.

Over-exertion (Not Elsewhere Classified)

On 2/25/15 Vulcan was informed by its third-party insurance administrator that an agreed medical evaluator through the litigation process determined the employee's post-termination alleged cumulative trauma injuries to his head, shoulders, arms, elbows, hands, fingers, L-foot, toes, knee and thigh were work-related.
