On 6/13/25 employee received medical treatment in ER for a reopening of a surgical incision. While employee was inspecting a trench, the tripped on leveling rake and fell on right knee, reopening a surgical incision from a personal knee surgery. Employee required surgery, was released from the hospital on 6/15/25, placed on restricted duty on 6/16/25 which is being accommodated
On 05-29-25 employee alleges a strain to the left lower back when operating a CAT 769D haul truck. EE states that the initial left lower back strain occurred on 05-28-25. While the employee was attempting to move a pallet with a pallet jack. On 05-30-25 employee was treated by a MD for a strain to the left lower back, resulting in modified work activity.
Loader operator missed the last rung on the ladder while dismounting the loader, falling to the ground (Operator did not use three-points of contact, carrying pre/post-shift book in one of EE's hands).
ON 9-22-25 EMPLOYEE REPORTED HAVING A FOREIGN BODY IN EE'S (R) EYE FROM TORCHING HOLES ON A NEW CLAD INSTALL ON 9-19-25. THE FOREIGN BODY WAS REMOVED IN THE EMERGENCY ROOM ON 9-22-2025. EE WAS PRESCRIBED EYE DROPS AND WAS RELEASED TO FULL DUTY SAME DAY.
