Fall to the walkway or working surface

Employee was loading drill in quarry, walked around platform and slipped on snow covered ice, fell and hurt right shoulder and arm. EE didn't see doctor right away but pain continued to bother so ee went to doctor on 1-13-2017. Recommended restrictions of lighter duty - 10# lifting, pushing, pulling limit. Site able to accommodate - No Lost Time.

Employee was in a hurry, grabbed a bucket of bits off grover with one hand. Bucket was heavier than he thought and it jerked him down. He has pain in lower back, shoots down into left leg. He sought chiropractic treatment that night after work.

Employee was stepping up on the ladder of loader, pulled his lower back and hip out. Has pain in lower back and hip socket on right hip.

Employee was pulling steel out with steel puller. Steel went through top of steel puller into the air, landed on his left big toe, right before the steel tip, fracturing his left big toe.

Employee was shutting dust collector door, it was stuck. Co-worker kicked it to free it, but employee had his finger in door, cutting left index finger, requiring sutures.

Struck against stationary object

Employee used his foot to kick rock out from in front of dust cup and drill steel, accidently hit feed lever, steel hit his foot, bruising it. Quarry was shut down the week of July 2nd through July 6th, he returned to work July 9th, he continues to work. He needed a prescription. He had follow up appointment. No lost time.

Struck by flying object

Employee was driving grout truck on dusty road, something got in his right eye. Foreign object became imbedded requiring medical removal.

It was raining outside, employee stepped on saw track to hook cable, slipped with right foot and fell, landing on track. Hit his right eye and head on saw track. He has a cut above and below his right eye from his safety glasses, sutures required. He also has a sore right lower leg.

Employee got out of loader, bent down to pick up lunchpail, felt very sharp pain in neck and shoulder right side. Employee went to the doctor right away, doctor diagnosed as cervicle sprain/strain, took employee off work until 9-28-05 when he said he could return to light duty, which Cold Spring Granite is accommodating.

Employee was allegedly pressure washing loader, hose got stuck, while getting hose free, spray nozzle was spraying on his foot, heat burned left big toe. Employee returned to work, then on 9-8-05 could no longer wear his boot so is off work until 9-12-05

Employee was welding and when chipping slag, a piece flew in left eye, burning his tear duct. Employee saw a doctor and was prescribed ointment - he returned to work later that day - no restrictions.

EE WAS LIFTING BUSTER TO WEDGE STONE WHEN HE FELT A PAIN IN HIS LOWER BACK. HE SAW A CHIROPRACTOR THAT AFTERNOON AND RETURNED TO WORK ON 5-16-03 WITH RESTRICTIONS UNTIL 5-19-03.
