Over-exertion in lifting objects

Employee was wedging blocks, lifting 40 lbs, with bending and twisting. EE strained low back. Sought treatment from chiropractor. No lost time at this point.

Employee was helping a vendor (Austin Septic) pump out water tank, built up with silt. EE was pulling on the suction hose & felt rib pop. EE went to the chiropractor to get adjustment, x-ray taken, no fracture. No lost time.

Employee was carrying 5 gallon pail of pipe fittings (approx. 75#) down stairs. He slipped off bottom step, twisting his back. He didn't seek treatment immediately, but went to chiropractor at later date. No known restrictions, no lost wages, no lost time. Reported due to treatment by chiropractor.

Fall from machine

Employee was unloading 20' sealed container, was getting down out of container. He was sitting & sliding on his butt between the forks on the loader, going to put foot on bumper, missed and fell to ground. Left shoulder got yanked behind him so it hit ground first. Left shoulder strain, thought pain would go away and it hasn't. Sought chiropractic care on 6-23-2015. No lost time.

Employee was chiseling hard packed dirt off of the stone he was drilling, using a 5' crowbar. He was swinging/twisting to his right with the crowbar and his back gave out. He went to the chiropractor and returned to work later that morning. No lost time.

Employee was walking in quarry yard. He slipped on patch of snow covered ice & fell, injuring his right elbow, left shoulder & right knee. He didn't seek medical attention until 4-2-14 at which time he also complained of neck pain, saw a chiropractor (which is considered medical treatment) & obtained relief. No restrictions. No lost time.

Employee was loading a shipping container with granite blocks, squeezing his way around in tight places. When blocking up the side of the block, he twisted and pulled something in his lower back, right side. He sought chiropractic treatment and returned to work. No lost time.

Employee was standing behind a mill block that was 10' x 6' x 3'. Loader operator set another block down in front of the block employee was standing behind. When backing out, the second block tipped into block by employee, causing it to tip into employee, pinning him under the block. Caused crushing injury to lower right leg, between knee & ankle. Also possible pelvis fractures.

Struck against stationary object

Employee was getting into the quarry plow truck, wearing all his bulky winter garb and his helmet. When he stepped to get into the truck he hit his helmet/head on the top of the door frame and jammed his neck. He went to a chiropractor for a couple of treatments. He was released without restrictions. No lost time.

Employee was using a blow pipe to clean off an area for lift holes. He was holding pipe with his right arm, also doing some shoveling. He pulled muscle in right arm/shoulder area. He thought pain would go away, it didn't. He went to chiropractor the next day after work and then again the next week.

Employee was walking from office to lunchroom, stepped on a granite slab with a fresh 1"" snowfall and slipped, landing on his back, pulled muscle on left side of neck. Employee treated at chiropractor for a few treatments. No restrictions, no lost time.

Employee treated with chiropractor two times so far, has work restrictions of bending limited to 30?, lifting limited to 30#, up to February 17th. Employee is working - no lost time.
