A ROOF FALL OCCURRED ON THE INTERSECTION OF #43 X-CUT IN THE #3 ENTRY OF SW MAINS SECTION. THE FALLEN AREA MEASURED 25 FT IN LENGTH, 21 FT IN WIDTH, AND 18 FT IN HEIGHT. NO PERSONS WERE INJURE D AND NO EQUIPMENT WAS DAMAGED.
A FRICTIONAL METHANE IGNITION OCCURRED IN THE #3 FACE OF THE HALLAM SECTION DURING REGULAR MINING ACTIVITIES. NO PERSONS WERE INJURED AND NO EQUIPMENT WAS DAMAGED.
A ROOF FALL OCCURRED AT THE 136 INTERSECTION (G-FACE) OF OUR MAINLINE HAULAGE. THE FALL WAS APPROX. 20'WIDE X 40'LONG X 5'HIGH. THE AREA HAD BEEN PREVIOUSLY SUPPORTED WITH 4'CONVENTIONAL BOLTS . NO PERSONS WERE INJURED AND NO EQUIPMENT WAS DAMAGED.

A ROOF FALL OCCURRED BETWEEN #16-#17 X-CUT OF THE OBHG LONGWALL SECTION IN THE TAILGATE ENTRY. THE FALL MEASURED 25 FT LONG BY 16 FT WIDE BY 20 FT HIGH (TOTAL HEIGHT FROM MINE FLOOR). NO PERSO NS WERE INJURED OR EQUIPMENT DAMAGED.
A ROOF FALL OCCURRED AT THE INTERSECTION OF #19 X-CUT IN #1 ENTRY OF THE SOUTHWEST MAINS. THE FALL MEASURED 20 FT LONG BY 20 FT WIDE BY 12 FT (TOTAL HY FROM MINE FLOOR) NO PERSONS WERE INJUR ED OR EQUIPMENT DAMAGED.

THE LIVINGSTONE PORTAL ELEVATOR BECAME DISABLED DUE TO A BROKEN COMPENSATING CHAIN. THERE WERE NO PERSONS ON THE ELEVATOR AT THE TIME OF THE INCIDENT.

A FRICTIONAL IGNITION OCCURRED IN THE SOUTH MAINS NO. 1 ENTRY DURING REGULAR MINING OPERATIONS. NO PERSONS WERE INJURED AND NO EQUIPMENT WAS DAMAGED.
THE SOMERSET ELEVATOR DOORS AT THE BOTTOM OF THE SHAFT WOULD NOT CLOSE DUE TO A LOOSE BOLT IN THE DOOR DRIVE MECHANISM THIS IN TURN CAUSED THE ELEVATOR TO STAY AT THE BOTTOM LOCATION.
AN IGNITION OCCURRED ON THE SOUTH WEST MAINS SE CTION INT HE ""0"" FACE WHILE MINING WAS GOING ON. NO PERSON WERE INJURED. THE FACE CONSISTED OF COAL AND ROCK 0.6% METHANE WAS REPORTED PRESENT.
EE WAS OPERATING A STAMLER COAL HAULER AND HAD ATTEMPTED TO TRAM AROUND AN UN CUT CORNER. IN DOING SO HIS HAULER APPARENTLY PINCHED THE ENEGIZED LOADER CABLE BETWEEN THE HAULER AND THE COAL RI B. EE EXITED THE MACHINE TO LOOK AT THE DAMAGE TO THE CABLE AND WAS APPARENTLY ELECTROCUTED WHEN HE CONTACTED THE FRAME OF THE COAL HAULER.
EE WAS OBSERVING A HAULAGE MOTOR ""COUPLE-UP"" TO A SUPPLY CAR. HE WAS RESTING HIS HAND ON THE SUPPLY CAR WHEN THEY COUPLED & THE IMPACT CAUSED THE ROOF STRAPS ON THE CAR TO MOVE CATCHING HIS RIGHT RING FINGER BETWEEN THE CAR & THE STRAPS.
EE WAS WALKING TOWARD THE MINER AND AS HE DUCKEDUNDER THE VENT TUBING HE WAS STRUCK IN THE BACK BY COAL AND ROCK THAT FELL FROM THE RIGHT RIB.
EMPLOYEE WAS STANDING AT THE HEADGATE CONTROL PANEL AND WAS STRUCK BY A PIECE OF FALLING ROOF COAL THAT FELL OUT FROM BETWEEN THE ROOF STRAPS.
EE WAS WORKING TO PULL EXCESS SLACK OUT OF THE FACE CONVEYOR CHAIN AT THE LONGWALL. IN THE PROCESS, THE CHAIN OF THE COME A LONG BEING USED TO PULL THE SLACK BROKE, CAUSING A PIECE OF THE CHAI N TO STRIKE HIS RT. RING FINGER.
EE WAS DIAGNOSED WITH TWO BULGING DISKS IN THE ""L"" REGION OF HIS LOWER BACK WHICH ARE REPORTED TO BE RELATED.
EE STATED THAT HE WAS HELPING TO RE-RAIL A JEEP ON THE TRACK HAULAGE. AS HE WAS LOWERING THE LIFTING JACK, THE JACK LEVER SPRUNG BACK FORCING THE JACK BAR FROM EE'S HANDS. THE END OF THE JACK BAR STRUCK HIM ON THE LEFT EAR.
EE CLAIMED THAT WHILE ROOF BOLTING ONT HE LEFT SIDE OF THE MINER/BOLTER HE PINCHED HIS LEFT INDEX FINGER. THIS OCCURRED AS HE WAS TIGHTENING THE LOWER HALF OF THE ROOF BOLT INTO THE TOP HALF W ITH COLLAR. HIS FINGER GOT PINCHED BETWEEN THE TOP OF THE COLLAR AND THE UNDERSIDE OF THE CHANNEL SLOT.
EMPLOYEE WAS OPERATING A JEEP ON THE MAINLINE HAULAGE AND WAS STRUCK BY A PIECE OF FALLING ROOF ROCK.
EE WAS WALKING & SLIPPED ON A PIECE OF CONVEYOR BELT. HE FELL TO THE GROUND & STRUCK HIS RIGHT KNEE ON A PIECE OF ROCK.
EMPLOYEE WAS HELPING TO ROOF BOLT ALONG THE LONGWALL FACE. HE WAS IN THE PROCESS OF REMOVING THE DRILL STEELS OUT OF THE PREVIOUSLY DRILLED HOLE WHEN HE WAS STRUCK ON THE HEAD & LEFT SHOULDER BY A PIECE OF SANDROCK THAT HAD FALLEN THROUGH THE ATRS ON THE BOLTING MACHINE.
ON 1-2-02 EE WAS ATTEMPTING TO RE RAIL A SUPPLY CAR USING AIR BAGS AND LIFTING JACKS. HE HAD INSTALLED AND INFLATED THE AIRBAG AND WAS ATTEMPTING TO MOVE THE CAR WITH THE JACK. WHEN THE CAR SH IFTED THE AIR BAG DISLODGED AND STRUCK HIM ON THE RIGHT LEG. EE CONTINUED WORKING THRU-OUT THE MONTH OF JANUARY BUT THEN BEGAN MISSING WORK ON 2-5-02.
EE STATES THAT ON 1-24-02, HE WAS WORKING AS A ROCK DUSTER AND AS HE WAS DRAGGING THE HOSE HIS LT. FOOT BECAME STUCK IN THE MUD AND AS HE PULLED FREE HE FELT A PAIN IN HIS LOWER BACK. HE COMP LETED HIS SHIFT AND THEN WORKED HIS NEXT SCHEDULED SHIFT ON 1-28-02, THEN BEGAN MISSING WORK ON 1-29-02.
EE WAS HELPING INSTALL ROOF BOLTS, AS HIS CO-WORKER WAS LOWERING THE BOLTER BOOM, EE REACHED INTO A PINCH POINT TO PULL A HYDRAULIC HOSE OUT AND CAUGHT HIS LT. HAND IN THE PINCH POINT. EE FIRST DAY AWAY FROM WORK WAS 1-26-02.
EE WAS HELPING TO REINSTALL THE BATTERY LID ON THE SCOOP BATTERIES. AS THE LID SLID INTO PLACE,IT PINCHED HIS RT. RING FINGER BETWEEN THE BATTERY CASE AND THE LID.
EE WAS HANDING CEMENT BLOCKS UP TO A SECOND EE FOR THE CONSTRUCTION OF AN OVER-CAST. A THIRD EE CUT A CEMENT BLOCK INTO PIECES ONT OP OF THE OVERCAST AND A PIECE OF THE BLOCK FLEW DOWN AND STR UCK THE INJURED EE ON THE LEFT THUMB.
EE WAS CENTER BOLTING IN #1 ENTRY OF HALLAMAN SECTION. AS HE WAS RETRIEVING DRILL STEEL FROM THE HOLE, THE TOP PORTION OF A DRILL STEEL SEPARATED & FELL. THE STEEL STRUCK HIS LEFT RING FINGER.
EE WAS GREASING THE BEARING OF THE SLOPE BELT BEND PULLEY. HE WAS HOLDING THE GREASE GUN WHEN SLACK FROM EXTENSION HOSE CAUGHT ON THE PULLEY LUG. HE PANICKED AND TURNED TO RUN WHEN HE STRUCK H IS HEAD ON A SUPPORT BEAM. ALSO RIGHT SHOULDER.
EMPLOYEE'S FACE WAS SUCKED INTO VENTILATION PIPE (FAN). SAFETY GLASSES BROKE HIS NOSE AND LACERATIONS TO NOSE, FACE AND LIP. ALSO CRACKED SOME TEETH.
EE WAS DISCONNECTING A 2"" RETURN HOSE OFF A LW SHIELD. HE WAS USING HIS PICKHAMMER TO HOLD THE HOSE FITTING WITH HIS LEFT HAND. HE WAS SWINGING ANOTHER HAMMER WITH HIS RIGHT HAND WHEN THE HOSE BROKE FREE. THE END OF THE HOSE FELL STRIKING THE UNDERSIDE OF HIS LEFT WRIST CAUSING THE INJURY.
EE WAS HELPING TO PLACE ROOF BOLT STRAPS ONTO THE CONTINUOUS MINER. AS HE SLID THE STRAP INTO PLACE A CO WORKER PULLED FORWARD ONT HE STRAP CAUSING EE TO PINCH HIS FINGER BETWEENT HE HOLE INT HE STRAP AND ANOTHER STRAP.
THE OPERATORS INVESTIGATION DETERMINED THAT THIS ALLEGED INJURY/ILLNESS WAS NOT WORK RELATED. THIS 700-1 IS ONLY BEING SUBMITTED TO MSHA IN ORDER TO ABATE CITATION NO. 7083284 ISSUED ON 2-14-0 3. THE SUBMISSION OF THIS 700-1 IS NOT AN ADMISSION OF ANY WORK DOING BY THE OPERATOR EE ALLEGES THAT HE SUFFERED AN OCCUPATIONAL ILLNESS DUE TO WORKING IN A DUSTY ATMOSPHERE.
EE WAS DIAGNOSED WITH A ""DEGENERATIVE RT. KNEE MENISCUS TEAR"" WHICH IS WORK RELATED.
EE WAS DIAGNOSED WITH CARPAL TUNNEL SYNDROME OF THE LEFT WRIST.
