Accident type, without injuries

Roof fat on track entry at 160 stopping 4 blocks outby spad 67 & 60. Fall 17 ft wide - 25 ft long - 8 ft high.

Livingston elevator is out of service due to a malfunction of dolly wheels on counter weight. Counter weight came free spinning around in shaft. Elevator will be returned to service after repairs and inspection of system.

Roof fall occurred on track entry of 33 mains between crosscuts 112 & 113. Fall was 50' long, 20' wide & 10' high. Ventilation was not impaired. No equipment was damaged and no people injured.

Employee was working at slope tail he went to z-convoyer to clear material. Loose coal/mud was being washed down slope by employees. Mud, coal and water slurry caused employee to loose his balance. He fell on his side and became stuck. Additional material was washed down slope partialy covering employee. Co-workers had to assist in removing employee from the area.

Struck by falling object

Drill steel fell out of drill hole stirking employee on left hand.

Employee slipped cleaning a windshield catching himself with his left arm. Later he felt pain in his shoulder.(Employee started missing time on 12/27/2005).

Employee slipped falling down steps at the load out.

Struck by falling object

Emoloyee was lifting his tailgate on his truck it slipped out of his hand striking his right knee. He started missing work on 10/20/05.

Over-exertion in lifting objects

He was changing a 25hp pump and as he pulled it over the hand rail he felt a pain in his right groin. Hernia was diagnosed on 8/30/05. Surgery is scheduled for 9/6/05.

WALKING IN #2 ENTRY OF 6-B SECTION. TRIPPED AND FELL TWISTING HIS LEFT ANKLE.

Lifting draw bar after coupling it up, he started to get up and felt pain in lower back.

Employee was tramming bolter from front controls. He got his SCSR stuck in the controls causing the bolter to turn into rib, pinching him.

Fall from machine

LOST HIS FOOTING COMING DOWN FROM TOP OF MOTOR. FELL AND CAUGHT HIMSELF WITH LEFT HAND CAUSING HIS LEFT THUMB TO BE OVER-EXTENDED.

Struck by falling object

Employee was standing by the miner when a piece of rock fell from between the channels, striking him on his right shoulder & head.

Over-exertion in pulling or pushing objects

Employee pushed a bottom roller under the belt. He stated that he felt pain in his left arm & something popped in his arm.

Helping to move monorail, one of the trolleys became stuck as he worked to free it. The trolley moved causing him to fall.

Employee was pulling cable using a cable roller hanger. The hanger broke striking him on the left shin.

Hit middle finger of left hand with a hammer he was using to tighten a block wall. Fractured tip of middle finger medical only no lost time.

Employee was changing brake shoes an a locomotive. The brake shoes slipped, pinching his little finger on left hand.

Employee was recovering ventilation tubes. In the process one fell on top of the fan tail tube causing it to deflate. When it inflated it caused the vent tube to pop up striking him on the jaw causing a laceration inside his mouth requiring sutures.

Employee was attempting to move cart containing oxygen/acetylene bottles. Cart caught on floor obstruction. Fellow employee pushed cart pinching injured employee's right hand between oxygen bottle and chute.

Employee complained of pain in left arm, shoulder and wrist. Does not have a specific incident. His arm has pain after work doing normal job. Started missing time 11-4-05. Diagnosed with epico ndylitis.

Employee experienced pain in his left elbow and was diagnosed with lateral epicondylitis. He has experienced pain for some time with elbow/forearm doing his regular job.

EMPLOYEE EXPERIENCED PAIN IN RIGHT ELBOW/FOREARM AFTER WORKING AS A BELT CLEANER. HE WAS DIAGNOSED WITH LATERAL EPICONDYLITIS (TENNIS ELBOW) on 8/30/05.
