Elevator at Somerset Portal stopped while ascending entrapping an individual from 1:50 PM until 2:50 PM
Elevator at Somerset Portal damaged, two wires shorted in a relay for the dynamic brake causing the elevator to stop while ascending.
Employee hit elevator doors with a hand cart while taking equipment off of elevator, knocked the cage door off track while at surface landing, causing the elevator to become inoperable.
A roof fall was discovered in the left wing, intake side of Zedicker shaft. The fall was above the normal anchorage zone. There were no persons injured or equipment damaged as a result of the roof fall.
The Livingston Portal elevator ceased operation at 7:45 AM on it's way up the shaft from the shaft bottom. There were 10 men that were riding the elevator at the time. The elevator was able to be brought to the surface at 8:45 AM and the men were safely removed. There were no injuries as a result of this incident.
A roof fall occurred at #92 Cross-cut along 33 Mains track haulage. This was a complete failure of the intersection. There were no persons injured or equipment damaged in this accident.
On 5/13/2008 there was a frictional/methane face ignition in the 10B Corridor, 082MMU mining section. There were no injuries to persons or equipment damaged as a result of this accident.
Employees became stuck on the Livingston Portal Elevator shortly after leaving the Surface landing en route to the mine level. This occurred at 12:05AM. The employees were able to get back to the surface at 12:45AM and everyone got out safely. The elevator contractor made the necessary repairs and the elevator was put back into service at 8:00AM on 1/28/08.
A roof fall was found at evaluation point ""D"" inby #101 cross-cut in #5 entry of the 33 Mains headings. There were no persons injured nor was there any damage to equipment as a result of this occurrence.
Employee got finger caught between frame of scoop bucket and hydraulic ram in scoop bucket, resulted in fractured distal phalanx right middle finger.
Employee was removing safety chains from between supply cars on slope bottom when he slipped striking his right jaw on something (undetermined) resulting in fracture to right jaw. Incident was not reported until 10-15-2008, began missing work on 10-16-2008.
Employee was removing a modulating valve from rock dust tank car. The valve fell off pinching his left index finger between the valve and a reflector bracket that was mounted on the car. Fractured left index finger.
Employee was helping to remove 2 joints of concrete pipe while pouring cement for belt pad. One of the pieces fell on his right foot. He started missing work on 7/5/2008 due to pain in right foot.
Employee was re-railing a trolley jeep. He raised the jeep with a lifting jack. While retrieving another jack from the vehicle, the set jack kicked out causing the jeep to fall. The top of his left foot became caught under the frame.
On June 2, 2008 employee was lifting one end of steel beam when his artificial left hip came out of socket causing him to fall on his left side injuring his left hip and groin area. He was being evaluated by a doctor until June 20, 2008. On June 20, 2008 he was evaluated and diagnosed as being unable to return to work.
Employee was dismantling guard gate of rail car, top half of the gate fell pinching the tip of the middle finger on his right hand.
On 5/21/2008 the employee was pulling cables through stopping and felt pain in left groin area. He has continued to work until 7/9/2008 when he started missing due to pain in groin area.
On 4/30/2008 the employee was pushing over an old cribbing that was set. In the process, he stepped on some material and slipped twisting his right knee. He has continued to work until 7/14/2008 when he started missing due to having surgery on his knee.
Employee was helping to carry a belt roller frame along the main line belt. He stated that he felt something snap resulting in pain to his right ankle.
Employee was helping to install face conveyor chain in Longwall panline. One of the flights got stuck as it was being pulled in. The employee was using a rock bar to pry the flight free. As he did this, the bar slipped and he fell causing lacerations to his nose and neck.
On 3/17/2008 the employee was stepping down the steps of a ladder on a belt cross-over. His foot became caught in the step causing him to fall to the bottom twisting his back. He continued to work until 3/26/2008 at which time he started missing due to back pain.
Employee was crawling across a conveyor belt cross-over platform. The left handrail broke causing the employee to strike the left side of his face on the frame of the platform.
Employee was helping to hose down the floor surrounding the stacker belt tailpiece. As he went to shut water hose off, he slipped on the expanded metal flooring straining his right shoulder and left knee. Employee has continued to work until 4/21/2008 when he started missing work due to pain from his injuries.
Employee was working to change bits in the longwall shearer. A piece of rock broke out between the face and roof. It fell striking him on the left knee. He has continued to work until 4/15/2008 when he started missing due to pain in the left knee.
On 2/8/08 the employee was using a bar to hold a spray block in place on the miner while helper secured it with a bolt. The employee has continued to work until 3/12/2008 when he started missing due to back pain.
EE was working as a roofbolter on the miner bolter. A piece of rock broke out of the roof, as he jumped out of the way of the falling rock he felt pain in his groin area. EE was diagnosed with a herina.
On 1/19/2008 the employee was stepping down off the stageloader at the longwall. He felt something pop in his left knee. He has continued to work and started missing time on 5/9/2008 to have surgery on the left knee.
On 1/11/08 the employee was working to re-rail an off track mine car. As he was prying the wheel hub, the car accidently moved causing the bar he was using to jar his left arm & shoulder. He continued to work until 2/29/08 when he started missing work due to pain in left shoulder.
While roofbolting, the employee was retrieving 3 sections of drill steels that were in the drill hole. In the process, the top section of the drill steel seperated and fell, striking him on the back of his right hand.
Employee was bolting on the left side of the miner, placed his left hand on top of rib protector to help push it out against the rib, rock fell from the roof pinching his fingers between the rock and rib protector. Resulted in 5 sutures in palm side of left index finger.
Employee was getting out of compartment after putting motor on charge. He struck his left ear on a fire suppression spray in the process, which required 5 sutures to close the laceration.
Employee was operating a trolley jeep on main haulage. The pole jumped off wire and struck overcast. As employee reached to get pole, it bounced back striking him in mouth and knocking out front tooth.
Employee was helping to unload vent tubes from bucket of scoop. He placed his right hand inside one of the tubes. The tube accidentally slid against the scoop bucket causing a laceration to his ring finger on his right hand.
Employee was diagnosed with Carpal Tunnel Syndrome on 8-25-08, employee started missing work on 9-5-08 due to his condition.
Employee was diagnosed with Left Carpal Tunnel Syndrome and began missing work on 1/8/08.
