LIVINGSTON ELEVATOR WAS OUT OF SERVICE FOR MORE THAT 30 MINUTES DUE TO MALFUNCTION ON OVERLOAD SWITCHES. ELEVATOR WAS DOWN WITH AN EMPLOYEE ON IT - FROM 3:40 PM TO 4:25 PM. SCHINDLER ELEVATOR SERVICE DID NECESSARY ADJUSTMENTS ON ELEVATOR AND INSPECTED IT THEN RETURNED IT TO SERVICE.

THE HALLAM ELEVATOR WAS OUT OF SERVICE FOR MORE THAN 30 MINUTES DUE TO THE POWER CABLE TO THE ELEVATOR CATCHING A WALL BRACKET & PULLING OUT OF JUNCTION BOX ON CAGE. 20 EMPLOYEE'S WERE ON THE ELEVATOR AT THE TIME. CABLE WAS REPLACED & GUARDWIRE INSTALLED TO KEEP CABLE AWAY FROM WALL BRACKETS. ELEVATOR WAS RETURNED TO SERVICE 11/11/04 AT 4:30 AM.

Livingston elevator door switch malfunctioned also the limit switch was out of alinement-repairs were made and elevator was replaced and tested then returned to service.

THE HOLLAM ELEVATOR DOOR SWITCH MALFUNCTIONED AND HAD TO BE REPAIRED. ELEVATOR WAS PUT BACK INTO SERVICE AFTER SWITCH WAS REPLACED.

THE SOMERSET ELEVATOR DOOR MOTOR MALFUNCTIONED AND HAD TO BE SENT OUT FOR REPAIRS. ELEVATOR WAS PUT BACK IN SERVICE 9/7/04 AFTER MOTOR WAS REPAIRED.

A ROOF FALL WAS DISCOVERED DURING A WEEKLY EXAM OF THE 2B BLEEDER ENTRY. IT OCCURRED AT #52 CROSSCUT, SPAD/#94+41. NO PERSONS WERE INJURED & NO EQUIPMENT WAS DAMAGED. NORMAL VENTILATION WAS NO T IMPAIRED.

A ROOF FALL OCCURRED IN THE TAILGATE ENTRY OF THE 3 BLW, SPAD #90+40. NO PERSONS WERE INJURED AND NO EQUIPMENT WAS DAMAGED. VENTILATION WAS NOT INTERRUPTED.

THE LIVINGSTON PORTAL ELEVATOR BECAME DISABLED DUE TO A SHORTED WIRE IN THE JUNCTION BOX THAT CONTROLS THE EMERGENCY STOP AND OVERIDE. NO PERSONS WERE ON THE ELEVATOR AND NO INJURES RESULTED.

THE ZEDIKER PORTAL ELEVATOR BECAME DISABLED WHEN THE DOOR LATCH SWITH BROKE. NO PERSONS WERE ON THE ELEVATOR AT THE TIME OF THE ACCIDENT AND NO PERSONS WERE INJURED.

A roof fall occurred on the tailgate of the 3 BLW. No persons were injured and no equipment was damaged. Ventilation was not interrupted.

THE LIVINGSTON ELEVATOR BECAME DISABLED DUE TO A BLOWN FUSE. TWO EMPLOYEES WERE STUCK FOR 15 MINUTES UNTIL REPAIR WAS MADE. NO INJURIES RESULTED FROM THIS ACCIDENT.

1/16/05 EE DID NOT SPECIFY A SPECIFIC INCIDENT. HE STATED THAT AFTER WORKING INSTALLING TROLLEY WIRE OVERHEAD, P1/SHIFT THAT HE FELT TIGHTNESS IN SHOULDER. HE STATED HE FELT IT WAS FROM WORKING OVERHEAD FOR 10-12 YRS AS A WIREMAN. WAS DIAGNOSED WITH A TORN ROTATOR CUFF.

Employee was helping to remove monorail from 3-B beltline. as he was dropping a section of monorail off the belt the flange struck his left wrist.

Ee while operating a scoop ran over a crib block. As scoop came off block it jarred the ee causing him to hit head on canopy of scoop.

Ee was helping to take a slider tube down from an idle entry. A piece of rock fell from between the roof straps striking the slider tube, which caused the tube to forcefully strike ee.

Ee was carrying a tube to miner. A rock fell from the roof and struck ee on back of lower right leg. Ee did not start missing work until 7-12-04.

EMPLOYEE WAS UNLOADING A SPOOL OF CABLE, WHILE PUSHING HE FELT A PAIN IN HIS LOWER BACK.

EMPLOYEE STATED THAT HE WAS ROOF BOLTING AT THE MINER. HE SAID THAT UPON COMPLETING DRILLING A HOLE FOR A ROOF BOLT, A PIECE OF ROCK BROKE OUT OF THE ROOF AS HE WAS RETRIEVING THE DRILL STEEL. THE ROCK FELL STRIKING HIS LEFT ELBOW.

Employee was turning on the water valve for the spray bar of the longwall shear. In the process, the lid on the compartment that the valve was housed in fell pinching the tip of middle finger on rt hand.

EMPLOYEE AND CO-WORKERS WERE CHANGING SHEARER JACK ON MINER. AS THEY WERE PUTTING THE NEW JACK IN PLACE, AND ATTEMPTING TO LINE UP BOTTOM EYE, THE JACK MOVED AND STRUCK THE TOP OF HIS LEFT HAND CATCHING IT BETWEEN THE JACK AND MINER PLATE.

EE WAS LIFTING THE BELT OFF OF THE SNUB ROLLER WITH A SET OF COMEALONGS HOOKED TO A PIPE THAT HAD BEEN PLACED UNDER THE BELT. THE EYELET IN THE PIPE THAT THE COMEALONGS WERE HOOKED INTO FAILED, ALLOWING THE PIPE TO KICK OUT & STRIKE EE ON THE ARM.

EE was reaching on top of the roof bolter and was struck on the arm by a piece of roof rock that fell from between the straps.

EMPLOYEE WAS CLIMBING DOWN LADDER FROM A FRONT LOADER AND MISSED THE BOTTOM STEP. HE FELL TO THE GROUND TWISTING HIS LEFT KNEE. HE WAS DIAGNOSED BY PHYSICIAN ON 3/31/04 FOR AN ACL TEAR ON LEFT KNEE.

Employee was walking and he slipped on a piece of wood in the walkway, causing him to fall onto his left knee.

EMPLOYEE WAS OPERATING TAILGATE DRUM OF THE SHEARER AND WAS MINING TOWARD THE TAIL. AS THE DRUM PASSED THROUGH AN AREA WHERE THE ROOF HAD PREVIOUSLY FALLEN 12' TO 14' HIGH, A PIECE OF ROCK WAS PICKED UP BY THE DRUM AND WAS THROWN INTO THE WALKWAY, STRIKING THE EMPLOYEE'S RIGHT LEG.

HE HAD JUST FINISHED MARKING A LOOSE ROOF BOLT IN AN OUTBY AIRWAY AND AS HE WAS WALKING AWAY A PIECE OF ROCK FELL OUT AND STRUCK HIS RIGHT LEG AND KNEE. HE SUFFERED AN ABRASION.

EE WAS SITTING ON BENCH IN BATH HOUSE PRIOR TO THE START OF HIS SHIFT AND LEANED OVER TO PUT HIS BOOTS ON, WHEN HE WAS STRUCK ON THE TOP OF THE HEAD BY A BASKET THAT DROPPED UNINTENTIONALLY. T HE BASKET WAS ATTACHED TO THE CABLE BUT APPARENTLY HAD NOT BEEN SECURELY FASTENED INT HE UP POSITION.

Two employees were cleaning of top of miner. As they were pushing on a large rock it slid pinching one employee's left middle finger.

EMPLOYEE PICKED UP A ROCK FROM WALKWAY ALONG BELTLINE. AS HE TURNED TO THROW THE ROCK ONTO THE BELT, HE STRUCK HIS LEFT HAND ON A BELT ROLLER FRAME.

EE WAS STANDING ON TOP OF A SUPPLY CAR HELPING TO UNLOAD CEMENT BLOCK. A CHAIN WAS ATTACHED AROUND 2 PALLETS OF BLOCK. AS HE WAS WALKING ACROSS CAR TO GET IN THE CLEAR HIS FOOT GOT CAUGHT IN HOLE OF THE CAR DECKING CAUSING HIM TO FALL TO THE MINE FLOOR STRIKING HIS MOUTH ON A ROCK.

Employee was walking through a cross cut on shaft bottom toward the regulator. As she got close to regulator, air current pulled her hard hat down. Safety glasses pushed on left cornea and bridge of nose, causing laceration.

EE WAS PLACING A CRIB BLOCK UNDER THE BACK OF THE MINER BUMPER AND HE INSTRUCTED THE MINER OPERATOR TO RAISE THE STAB JACK, THE OPERATOR DID AS INSTRUCTED AND EE'S RIGHT THUMB WAS PINCHED BETW EEN THE MINER FRAME AND THE CRIB BLOCK.
