Accident type, without injuries

The Livingston Portal elevator stopped in the shaft during an inspection. There were 2 employees on the elevator at the time. The elevator repair contractor was called immediately and the elevator was back in service by 10:15 AM. There were no injuries as a result of this event.

An employee became stuck on the Hallam Portal elevator upon entry into the mine. The elevator stopped about 70 ft. down from the surface landing. The elevator service contractor was immediately called and was able to get the elevator back in service by 2:45 AM. There were no injuries as a result of this incident.

A roof fall occurred on 33M track haulage at #80 crosscut spad #64+97. No persons were injured or equipment damaged as a result of this occurrance.

The Somerset Portal elevator stopped operation when a linkage arm for the bottom gate elevator doors came off the switch. The elevator service contractor was called, repairs were made, and the elevator was put back in safe operation at 9:00AM. There were no persons injured, or trapped as a result of this incident.

It was reported that there was a slight depression in the breastworks of the #8 Sediment Pond for the Prep Plant. Heavy rainfall has contributed to this condition. The breastworks have not failed, and there is no immediate danger to other persons or property. There was nobody injured due to this incident.

The Somerset Portal elevator doors failed to close at the mine level. This was due to the cable slipping off the door pulley. The elevator repair service contractor was called, made the repair, and placed the elevator back in service. There were no persons injured or entrapped due to the incident.

On 7/24/07 a roof fall was discovered by a belt cleaner at #151 cross-cut along Somerset Track haulage. No persons were injured and no equipment was damaged

The Hallam elevator stopped 20ft. off the mine level bottom as it was traveling back to the surface due to a tripped governer switch. There were no persons on board the elevator at the time. The elevator service contractor was called immediately. The elevator was put back into service at 2:45AM.

Four men became stuck on livingston elevator, while trying to exit the mine on 6/24/07 at 2:23 pm. The elevator started ascending as normal, stopped 2' above mine level (bottom). Maintenance forman opened doors and all men exited elevator at 2:33pm. Investigation-door cable caught, causing elevator to stop.

Disruption of power supply because of electrical storm. Caused elevator to stop for about 15 mins. with 5 men on it. Back up generator was started & elevator was put back into service.

Roof fall occurred at 111 crosscut on (Somerset track) South Mains Haulage. Fall is 24 feet long, 8 feet wide and 8 feet high.

The bottom landing door on the Livingston elevator was damaged by ice. The elevator itself was not damaged.

Elevator was put on hold by the operator as a precaution till it could be checked by the elevator service. The elevator was not out of service because of failure.

The Livingston elevator became disabled due to a damaged bracket on the govener rope. 3 men were stuck on the elevator.

Roof fall occurred in 8 butt in the number 2 entry at 17 crosscut. Fall was 9 feet high, 16 feet wide and 22 feet long.

Struck against stationary object

The victim was standing behind the center bolter observing the slack power cable laying on the ground. As he walked around the bolter to check for slack, he stepped on a small rock (saucer dish size) causing him to twist & fall to the bottom. He was taken to the hospital and a soft cast was put on. He went into convulsions on 4/28/07 at home and later died at the hospital.

On 11-23-07 employee was dragging rockdust hose and fell backward on his right elbow. He has continued to work until 1-18-2008 when he started missing due to pain in right shoulder.

Employee was standing alongside a bundle of steel roof props. He rested his right hand on the base of one of these props. The other end of the prop slid off the bundle which caused the base plate his hand was on to pinch the tip of his right finger against another base plate.

Employee was attempting to remove a dislodged roof bolt using a piece of rope between the bolt and drill head. He accidentally activated the rotation lever causing him to get his index finger caught between the rope and tightening wrench.

On 10/10/2007 the employee was climbing up on dozer at the Prep Plant. He slipped and strained his right shoulder in the process. He has continued to work until 10/29/07 when he began missing for pain in Rt. shoulder and neck area

Contact with electrical current

Employee was taking down a shuttle car cable that was hanging on insulated hook. He received an electrical shock from the cable. He started missing work on 10/09/07 due to this incident.

On 9-12-07 the employee was walking in entry and stepped in mud with his left foot. As he attempted to take his next step his left foot remained stuck causing him to twist his left knee. He continued working until 10/31/07 at which time he started missing work due to pain in left knee.

EE fell while cleaning bottom of thickner at the prep plant. He strained his left shoulder in the process. EE started missing work on 9/10/07 due to left shoulder pain.

Struck against stationary object

Employee was roof bolting on miner in 8b section. He stepped on some loose coal twisting his left ankle. Upon diagnosis by dr on 9/6/07 it was determined that he sustained a fracture.

On 9/5/07 EE was in the process of re-fueling a compactor. He strained his left shoulder while removing the fuel nozzls. He continued to work until 11/1/07 at which time he began missing work due to pain in lt. shoulder.

Fall onto or against objects

Employee was charging a scoop. He tripped on a water hose and fell striking his lower right leg on a charger cable. He began missing work on 9/7/07 due to pain in right leg.

While working as a longwall shieldman. He was advancing a lw shield that was dropped back. A pressure hose broke in the process and struck him on left side and knocked him down.

Struck by falling object

Employee was struck on the left shoulder with a piece of rock that fell from roof. This occurred on 8/15/07. He continued to work until 10/25/2007 at which time he started missing work due to pain in lt. shoulder.

Over-exertion in lifting objects

Employee was unloading belt rollers off supply car and felt pain in his right shoulder. The employee has continued to work until 4/25/2008 when he started missing work due to pain in right sho ulder.

Fall to the walkway or working surface

On 7/21/07 the employee slipped and fell while working in slope. He continued to work until 9/12/07 at which time he started missing work due to pain in rt shoulder.

Over-exertion in pulling or pushing objects

Employee stated that he was pulling on rockdust hose and felt pain in his groin area. He continued to work until 7/23/07 at which time he was diagnosed with a bilateral hernia.

On 7/10/2007 the employee was walking along a beltline. He stated that he slipped in some mud twisting his right knee. The employee has continued to work until 8/31/2007 at which time he started missing work due to surgery on his right knee.

EE complained of left knee pain after working on the longwall face. EE was diagonsed with degeneration to posterior horn lateral meniscus and medial meniscus , started missing time on 6/18/07.

Employee was using a chain fall to raise an 18"" pipe elbow into position on 6/10/07. He began missing work on 8/14/07 due to pain in rt shoulder.

On 6/09/2007 the employee fell while removing an acetylene tank out of a mantrip. He fell to the mine floor striking his right shoulder. The employee has continued to work until 8/30/2007, at which time he started missing work due to pain in right arm, shoulder and neck.

Pulling batteries onto scoop when hook slipped off batteries, wire rope recoiled and struck employee on the left cheek.

Employee was replacing liner plate in feeder chute. Liner shifted catching his left thumb. Diagnosed with a fracture. (Started missing time 4/26/07.)

Employee was trying to remove a ram jack. He caught the tips of his middle and index fingers between the ram jack and frame of thru conveyor.

Over-exertion in lifting objects

Placing trolley pole on wire, felt pain in abdomen. Diagnosed on 5/7/07 with an umbilical and bilateral hernia.

Employee caught right index finger between ram and frame of scoop pinching tip of his finger. (requiring sutures)

Over-exertion in lifting objects

Employee was lifting an ATRS pad and felt pain in groin area. He was diagnosed with a hernia and started missing time on 3/6/07.

Employee stated as he turned to walk away from work area he felt pain in left knee. He was diagnosed with a miniscus tear. Employee started missing time on 6/5/2007.

Struck by falling object

Employee stated he was standing alongside the Continuous Miner/Bolter. A piece of rock fell from roof striking him on the head and left index finger. Employee received 10 sutures in left index finger.

Employee was helping to install a pin into the pan on a continuous miner. As the pin was hammered, a piece of metal flew out striking employee in the right cheek. He received 3 sutures. Employee has not missed any work due to this incident.

Caught in, under or between a moving and a stationary object

Employee was getting out of company truck at the Prep Plant Office. The middle finger on his right hand got caught in the door frame as he closed the driver's side door.

Employee was diagnosed with bilateral tunnel syndrome and began missing work on 11/5/2007.

Employee started missing work due to continuing pain and numbness in his left hand. Upon medical evaluation on 6/29/07 he was dignosed with tenosynovitis.
