Roof fall on B belt at discharge outby first over cast. Fall measured 6ft high 25ft long 18ft wide. No injuries were reported as a result of this fall.
AN EE WAS ATTEMPTING TO DISCONNECT A FAIRCHILD 35-C SCOOP FROM THE CHARGING SYSTEM. WHILE DISCONNECTING THE CHARGING PLUG A SPARK OCCURRED FROM A GROUNDED LEAD CAUSING AN IGNITION OF HYDROGEN GAS THE HAD ACCUMULATED UNDERNEATH THE LIDS OF THE BATTERY. NO INJURIES WERE REPORTED IN CONNECTION TO THIS IGNITION.
WHILE TRAVELING THE RETURN AIRWAY A ROOF FALL WAS DISCOVERED 7 BRKS OUTBY R5 SEALS, ADJACENT TO E BELT LINE. THE ROOF FALL MEASURED 20 FT WIDE, 40 FT. LONG AND 3 FT HIGH. NO INJURIES WHERE REPORTED AS A RESULT OF THIS ROOF FALL.
Hoist at 1a portal taken out of service for routine schedule maintance.
A roof fall was discovered in the #6 entry at brk 9 in the intersection adjacent to the 6a belt measurements of the roof fall are 6' high 19' wide and 120' long. No injuries were reported as a result of this roof fall.
A roof fall occurred in the track entry, along the company no. 4c belt conveyor system. The fall is located at company no. 14 intersection, and measured approx. 20'x30'x6 to 7' high. The fall is approx. 6000 ft from the active working section company no. 002. There were no injuries reported in connection to the fall and no equipment trapped.
The hoist at #1 mine had slack in the rope at the drum. The hoist was down for more than 30 minutes while the rope was being tightened at the drum.
Break 15 one break inby spad 24902 in 3 entry a roof fall occurred in the enter section. Measurements of the roof fall are 5' high 20' wide and 22' long. No injuries were reported as a result of this roof fall.
Employee discovered a roof fall in the neutral break 11, along 7C belt 2 breaks out by spad 28814. Measurements on the roof are 18 ft wide, 18 ft long, and 6 ft high. This area has been cribbed and dangered off.

A ROOF FALL WAS FOUND DURING A ROUTINE PRESHIFT EXAM OF THE 0UTBY AREAS. THE FALL IS LOCATED AT INTERSECTION #42, TRACK ENTRY ALONG THE #2 BELT CONVEYOR. THE FALL MEASURES APPROX 7-10' LONG, 15-20' WIDE AND 4-6' HIGH. THERE WERE NO INJURIES TO REPORT IN RELATIONSHIP TO THIS FALL
While performing work on #3 track motor anderson plug that was grounded, employee decided to remove battery cover lid so that he could cut one of the leads going to the anderson plug. While using a pair of kline cutters, he cut one lead that resulted in an arc causing hydrogen gas to explode.
WHILE DOING PRESHIFT EXAM ON HOIST, FOUND BROKEN WIRE IN HOIST ROPE. HOIST WAS IMMEDIATELY TAKEN OUT OF SERVICE. HOIST ROPE HAS BEEN CHANGED OUT.
While traveling the return airway course, #2 roof falls were discovered int he L14 seal area in #7 entry #1 roof fall measure 5' high 18' in width 25' in length #2 roof fall meausre 5' in high 18' width and 20' in length. No injures were reported as an result of these roof falls.
Roof fall on MMU 001-0 section. 1-Z panel in #4 heading inby survey station 29025. The fall measured 14ft high, 20ft in length, 18ft width. No injuries were reported as a result of this fall.

Hoist down from 11:30 am to 4:30 pm. Coil in main contactor and surge suppriss burnt. repaired approximately 5:00 pm. Hoist down second time from 6:15 to 4:30 am in coil in main rectifier. No injuries reported as a result of this hoist being down.
A ROOF FALL WAS FOUND DURING A MINE EXAMINATION. THE FALL IS LOCATED FOUR CROSS CUTS OUT BY COMPANY #3 BETL DRIVE. ONE CROSS CUT OUT BY SURVEY SPAD #4371. THE FALL MEASURES APPROX 35' X 20' WIDE X 8' HIGH IN THE INTERSECTION AND EXTENDING INTO THE BREAK. THE AREA WAS IMMEDIATELY DANGERED OFF ON ALL SIDES. THERE WERE NO INJURIES REPORTED.
A roof fall was found during a mine examination on 02-02-2005 @ 9:45 pm. The fall is located 19th cross cuts inby Company No. 4C belt conveyor drive, or 1st cross cut inby Survey Spad No. 11713, Track Entry and cross cut between track and belt. The fall measured approximately 20' x 90' x 8' high. There were no injuries reported in relation to this fall.
A roof fall was found during a mine examination on 02-01-2005 @ 3:00 pm. The fall is located in the belt entry, 1st cross cut outby Spad No. 13202. Or 3rd cross cut inby Company No. 3C belt conveyor drive. The fall measured approximately 20' x 20' x 8' wide. There were no injuries reported in relation to this fall.

THE DISTAL DIGIT WAS SURGICALLY REMOVED LATER THAT DAY
EE was in the process of a roof fall rehabilitation. a piece of draw rock fell hitting ee on the left hand. (We have received Medical information that the ee first two joints of his left middle finger were surgically amputated)
EE WAS IN THE PROCESS OF ROOF BOLTING PROCEDURES IN THE #4 ENTRY ON COMPANY #001 SEC. HE HAD JUST FINISHED DRILLING HOLE TO INSTALL A 6' RESIN GROUTED ROD & WAS IN THE PROCESS OF REMOVING THE DRILL STEEL FROM THE HOLE WHEN A PIECE OF DRAW ROCK FELL STRIKING HIM ON THE LEFT LEG ABOVE THE ANKLE. THE ROCK MEASURED APPROX. 39""L X 24""W X 1/2"" TO 3 1/2""THICK. FRACTURED TIBIA & FIBULA.
LEAKING ACETYLENE TANK INSIDE CLOSED BOX, IGNITED BY TORCH, CAUSING LID TO FLY OPEN AND BREAK ARM.
After negotiate the curve adjacent to 7c head drive on a 10 ton track motor. EE stop to recouple the 10 ton track motor too the two supplys cars that other was pshing around the curve when ee was struck by the supply car in attempt to couple the draw heads together.
The employee stated he was in the last open cross cut between the no. 2 & 3 entry, on the company no. 004. continuous miner section. He was in the process of walking by the continuous miner in route to get his scoop, when a piece of draw rock suddenly fell striking him on the head, back and left knee area. The piece of draw rock measured approx. 1'x2.5'x4""thick.
THE EE STATED THERE WAS SOME DRAW ROCK LYING ON TOP OF A SHUTTLE CAR CABLE. HE VISUALLY EXAMINED THE ROOF, THEN PROCEEDED TO REMOVE DRAW ROCK BY HAND OFF CABLE. A PIECE OF DRAW ROCK FELL SUDDENLY STRIKING HIM ON HEAD & SHOULDERS KNOCKING HIM TO MINE FLOOR CAUSING A LACERATION BETWEEN LITTLE & RING FINGER LEFT HAND.REMOVING ROCK CAUSED HERNIA IN LOWER GROIN AREA.
After installing a curtain board for ventilation controls, a piece of draw rock fell between the permanent roof supports roof bolts. Striking EE on the shoulder and lower back. He missed work on the 10/10/05 threw 10/13/05.
While cutting 8 right break a piece of draw rock fell striking ee on the head resulting in a laceration on the head required sutures to close.

EE WAS IN THE PROCESS OF ROOF BOLTING PROCEDURES IN THE #6 HEADING ON THE #004 CONTINUOUS MINER SECTION. A PIECE OF DRAW ROCK (APPROX. 29"" X 30"" X 3"") FELL ON TOP OF THE CANOPY, THEN SLID OFF THE CANOPY HITTING THE EE ON TOP OF THE RIGHT FOOT CAUSING A FRACTURE OF THE FOOT & ANKLE. THE FRACTURES WERE NOT FOUND UNTIL A BONE SCAN WAS DONE ON 10/3/05.
WHILE SHOVELING AT SECTION TAIL PIECE FELT SHARP PAIN IN LOWER BACK
EE WAS IN THE PROCESS OF ASSISTING A SHUTTLE CAR OPERATOR ADD ADDITIONAL SLACK TO THE REEL OF SHUTTLE CAR. THE CAR OPERATOR WAS ADDING SLACK BY STARTING & STOPPING THE PUMP MOTOR OF THE SHUTTLE CAR. IN THE PROCESS OF ADDING SLACK, THE CABLE STRUCK INJURED IN THE KNEES KNOCKING HIM TO THE GROUND CAUSING A TORN CATRILDGE. HE CONTINUED TO WORK UNTIL SURGERY 10/21/2005
THE EE STATED THAT HE WAS IN THE PROCESS OF MOVING THE CONTINUOUS MINER CABLE & WATER LINE OUT OF HIS HAULWAY ON CONTINUOUS MINER SEC #002. DURING THE LIFTING OF THE MINER CABLE & WATER LINE HE FELT A SHARP PAIN IN HIS LOWER BACK. EE CONTINUED TO WORK UNTIL 10/10/05 WHEN HE WENT TO HIS FAMILY DR WHO ADVISED HIM TO DISCONTINUE WORK UNTIL HE COULD SEE A SPECIALIST LATER THIS MONTH.
The employee stated that while he was in the process of roof bolting procedures. He was pulling on some drill steel that was stuck int he drilled hole of the roof. The drill steel broke, throwing him off balance twisting his right knee. The employee stated that he hurt this same knee in a car wreck in the year 2000 but never could afford to seek medical attention at that time.
While loading 50 pound bags of rock dust, employee felt pain in left wrist.
While welding on the operators side light, rear area, of a Coalage ""488"" Scoop. A spark from the welding activity caused sudden movement of the battery lid, which caused fractures of the left eye orbit area.
Employee was walking through a break and tripped on a piece of draw rock, twisting his right knee in the process. He did not see the Dr. until the following week, 3 days later.
EE stated that he was int he process of loading rock dust onto the back of his scoop. He was lifting the dust and turning 90 to 180 degrees stacking the dust on top of the scoop. While in the process of turning with a bag of dust in his hand. He stepped onto a piece of rock which threw him off balance into the scoop. Twisting his right knee in the process.
WHILE HANGING MINER CABLE EE REPORTED EE FELT PAIN IN LOWER BACK
The EE stated that he was in the process of throwing over the miner cable out of the roadway. A piece of draw rock fell from between the pins and hit him on top of the head, then on top of his left foot. The accident occurred on the #001 section in #6 entry, 2nd cross cut outby the face.
EE was installing permanent roof support on a production unit when a small piece of draw rock fell striking him on the right side of the head. He saw his Dr and was scheduled for a CT scan an April 12, 05. After Dr reviewed the results of the CT scan he was advised to remain off work until April 21, 05.
EE WAS WALKING TOWARD A SCOOP TO RETREIVE A BUNGLE A CAP WEDGES AND STEPPED BETWEEN A ROCK AND TIMBER CAUSING EE TO FALL, TWISTING EE KNEE IN THE PROCESS. THIS REPORT IS LATE DUE TO WAITING ON REPORT FROM SPECIALIST.
Employee was installing permanent roof supports. As he was swinging the roof bolter boom out to install a roof bolt he twisted his knee. This report was delayed awaiting MRI and report from physician.
While re-entering a miner cable EE felt a sharp pain in his left knee when he slipped. Continual wear & tear has resulted in his undergoing arthroscopy for treatment of degenerative mensical tear and chondromalacia on 3-31-05. First day away from work 3-7-05 report delayed until investigation could be completed.
TO THE BEST OF MY KNOWLEDGE, EE RECEIVED A STRAINED BACK WHEN THE MANTRIP HE WAS RIDING IN STRUCK ANOTHER MANTRIP. REPORT DELAYED DUE TO LACK OF INFORMATION.
EE STATED WAS SHOVELING ON ""B"" BELT AT AIR LOCKS WHEN EE CAME IN CONTACT WITH THE BELT.
EE was setting timbers when he experienced pain in lower back.
EE LET ATRS DOWN AND LOOSE ROCK DISLODGED, FALLING AND HITTING HIM ON RIGHT FOOT.
The employee was holding a small piece of cable in his left hand and was using a pocket knife to cut the insulation from around the cable. The cable twisted in his hand while he was cutting with the knife, causing the knife to slip and creating an incision wound in the lower portion of the left hand, palm area. The wound required 6 sutures to close.
The employee was loading 6' resin grouted roof bolts by hand into a scoop bucket. During the loading process the bolts shifted and pinched his finger between the bolts. Causing a laceration to the right hand middle finger that required 12 sutures to close.
THE EMPLOYEE WAS WALKING ACROSS THECONTINUOUS MINER SECTION WHEN HE STUMBLED AND FELL. DURING THE FALL, HE FELL ON HIS PICK HAMMER THAT HE HAD ATTACHED TO THE SIDE OF HIS LIGHT POUCH. THE PICK HAMMER CAUSED A PUNCTURE WOUND TO THE RIGHT LEG, THICH AREA THAT REQUIRED 6 SUTURES TO CLOSE.
THE EE WAS UNLOADING A STEEL BAR FROM A UNDERGROUND VEHICLE TO THE MINE FLOOR. IN THE PROCESS OF PULLING THE BAR OUT OF THE VEHICLE HIS THUMB WAS PINCHED BETWEEN THE FRAME OF THE VEHICLE AND STEEL BAR CAUSING A SIMPLE FRACTURE OF THE THUMB. THE BAR MEASURED APPROXIMATELY 4' TALL BY 6"" WIDE BY 1"" THICK.
EE was in the process of getting ready too wedge a timber. While reaching down too pick up a bundle cap wedges, Other using a pick struck him on the right arm just above the right elbow resulting in a puncture wound that required 3 sutures to close.
EE was in the process of operating a battery powered. rubber tired four wheel out of his roadway when the steering wheel suddenly spun around. The steering wheel is equip.with a steering knob, which struck his right middle finger causing a fracture of the distal digit of middle finger, right hand. this report was delayed & waiting on dignostic results.
Employee had pulled a locker from inside warehouse to inside shop and was in the process of standing the locker upright when the piece of metal that he had his hand on broke off causing him to drop the cabinet locker on his left leg. Resulted in a laceration on his left leg that required 10 sutures to close.
EMPLOYEE WAS LOADING A STEEL BOX THAT MEASURED APPROX 1.5' WIDE X 2.5' LONG X 1.5' TALL IN THE BUCKET OF A SKID LOADER ""BOB CAT"". WHEN HE SLID THE BOX INTO THE BUCKET, HIS LEFT HAND INDEX FINGER WAS PINCHED BETWEEN THE BUCKET OF THE LOADER AND STEEL BOX CAUSING A LACERATION TO INDEX FINGER OF LEFT HAND. THE WOUND REQUIRED 10 STITCHES TO CLOSE.
While shoveling around outby end of 7B tail piece, employee was struck by a piece of rock that bounced off the conveyor belt, resulting in 5 sutures to close a laceration to the second digit on his right hand.
The ee stated that he was in the process of cleaning on company no. 6-b belt drive. He was int he process of getting on his knee's when the belt knife that was attached to his mining belt was unexpectedly opened and cut his left leg. The wound required 6 stitches to close.
Employee stated that he was in the process of adjusting a top belt structure by hand. He grabed the top roller and attempted to adjust it when, his little finger was caught between the structure and rail. This caused a laceration to the little finger, left hand. the wound required 15 stitches to close.
WHILE STANDING ON TOP OF CONTINOUS MINER THAT WAS PARKED HANGING A VENTILATION CURTAIN, EE STEP HIS LEFT FOOT BETWEEN THE END OF THE CONVER CHAIN AND THE BRACE AT THE END OF BOOM, LOST HIS BALANCE AND FELL WITH HIS LEFT FOOT STUCK CAUSING BOTH FIBIA & TIBIA TO BREAK
EE WAS PULLING ON 1 1/4 WATER LINE WHERE SOME OF IT WAS HUNG AND THE OTHER WAS LAYING IN MUD AND WATER IN AN ATTEMPT TO MOVE WATER LINE OUT OF THE WAY WHILE PULLING WITH HIS HANDS THE WATER LINE SNAPPED BACK AND STRUCK HIM IN UPPER LIP RESULTING IN LACERATION THAT REQUIRED 2 SUTURES
Employee stated he was on his way to work. As he was coming upon the gate at the entrance to the mine he thought the gate was up. He stated that he looked away briefly and ran into the gate which security cameras show was down. As he struck the security gate he was knocked off his motor bike. He has not missed any work.
Employee was making belt splice in 3f belt while using a utility knife to cut the belt. He was pulling the knife toward his self when he hit his right leg with the knife resulting in 9 sutures to close incised wound.
The employee was in the process of repair work of a roof bolting machine using a non-powered hand tool. The tool slipped causing a sudden impact of the left hand against the frame of the bolting machine, causing a cracture of the ring finger.
Whilw working on setting new head drive on #3 section employee was helping pulling new belt through drive rollers when a piece of draw rock fell striking him on left arm near his elbow resulting in 7 sutures to close.
WHILE MARKING THE CENTER LINE IN #1 HEADING, A PIECE OF DRAW ROCK FELL STRIKING EMPLOYEE IN THE FACE RESULTING IN AN INCISED WOUND ON THE BRIDGE OF THE NOSE THAT REQUIRED 7 SUTURES TO CLOSE.
EE STATED HE WAS IN THE PROCESS OF INSTALLING ROOF SUPPORT IN #7 ENTRY ON THE #001 CONTINUOUS MINER SECTION. APPROX. HALF OF THE CUT WAS BOLTED AND EE WAS SET UP ON THE 1ST BOLT OF THE NEXT ROW. WHEN HE STARTED DRILLING, A PIECE OF DRAW ROCK APPROX. 12"" X 8"" X 2"" THICK SUDDENLY FELL, STRIKING HIM ON THE LEFT HAND. LACERATION TO RING FINGER; 6 STITCHES TO CLOSE WOUND.

THE EMPLOYEE WAS ON HIS KNEES LOOKING UNDERNEATH A PIECE OF EQUIPMENT ON PRODUCTION UNIT #2 WHEN A PIECE OF DRAW ROCK MEASURING APPROX 1' X 1' X 1"" FELL KNOCKING HIS HAT OFF CAUSING A LACERATION TO THE TOP OF THE HEAD. IT TOOK 12 STITCHES TO CLOSE THE WOUND
Employee had both of his hands on a monitor pack that controls power for a miner on MMU 011-0 section when he received electrical shock from 110 volts. He did not miss any work as a result of this injury.
EE WAS IN THE PROCESS OF HANGING THE ROOF BOLTER CABLE WHEN THE OPERATOR TURNED THE CABLE REEL ON TO TAKE UP SLACK. THE CABLE STRUCK THE EE IN THE BOTTOM PART OF THE CHIN, CUTTING THE INSIDE OF HIS BOTTOM LIP. 8 STITCHES REQUIRED TO CLOSE THE WOUND.
EMPLOYEE STATED THAT HE WAS WALKING BESIDE A ROOF BOLTING MACHINE TO GET A ROOF BOLT AND CONTACTED A ROOF STRAP THAT HAD THE CORNER BENT DOWN. THIS PRODUCED A LACERATION TO THE RIGHT SIDE OF THE HEAD, TEMPLE AREA. THE WOUND TOOK 7 STITCHES TO CLOSE
EE STATED HE WAS GIRNDING A WEAR LINER IN A WASH BOX WHILE SEVERAL OTHERS WERE INSTALLING FLIGHTS IN THE WASH BOX. THE FLIGHT THEY WERE INSTALLING CAME OFF THE BOLTS CAUSING IT TO SWING AROUND. THE SWINGING MOTION CAUSED IT TO PULL OUT OF THE HOLDER ON THE OPPOSITE END AND FALL. THE SWINGING MOTION THREW THE FLIGHT OUT 4-5 FT AND IT STRUCK HIM ACROSS THE TOP OF THE HEAD
EE STATED THAT WHILE HE WAS DOING MINOR REPAIR ON A ROOF BOLTER, HE HAD HIS HAND RESTING ON BOOM OF THE BOLTER. A SMALL PIECE OF DRAW ROCK FELL BETWEEN THE BOLTS AND STRUCK HIM ON THE TIP OF THE MIDDLE FINGER, LEFT HAND.
Employee stated that he was hanging a curtain behind a stopping that needed repaired. EE bent over to pick up a hilti gun in order to nail the curtain to the roof. A piece of draw rock fell from around the roof bolt and cut the back of his head.
EE STATED THAT HE SWUNG HEAD TO CHECK SUCTION HOSE ON ROOF BOLTER. HE PLACED HIS HAND ON BOOM AND A 3"" X 3"" ROCK FELL, STRIKING HIS HAND. RECEIVED MEDICAL INFORMATION ON 1/24/05 THAT FINGER WAS FRACTURED.
EE STATED HE WAS HELPING PLACE SHIMS BETWEEN MOTOR AND FRAME OF SLOPE BELT SPEED REDUCER. SHIM FELL OUT AND STRUCK HIS FOOT. RECEIVED MEDICAL INFORMATION ON 1/24/05 THAT HIS TOE WAS FRACTURED.
Employee was removing rock from miner when a piece of rock fell from top and hit him on top of his right hand 12""x8""x1"".
THE EE STATED HE WAS AT THE REBUILD SHOP DURING WELDING CLASS TRAINING SESSION. HE WAS EXPOSED TO THE HAZARDS OF PLASMA CUTTING & MIG WELDING ACTIVITIES FOR THE DURATION OF THE ENTIRE SHIFT, 7:00AM TO 3:00PM. LATE THAT NIGHT HE EXPERIENCED BURNING SENSATIONS IN BOTH EYES. HE WENT TO DR. THE FOLLOWING DAY WHERE HE RECEIVED MEDICAL ATTENTION FOR OVER EXPOSURE TO WELDING ACTIVITIES.
While underground employee requested the hoistman transport him to the surface due to experiencing chest pains. Transportation took approximately 3 minutes. When employee arrived on the surface no vital signs could be detected. CPR was performed for 20 minutes before ambulance service arrived. The coroner informed management that the employee suffered a heart attack.
EE stated that he was getting out of the deck of a 488 scoop. He pulled himself out using hand hole cut outs on the canopy. In the process of exiting the deck he placed both feet on the back frame of the scoop deck. When he was out of the deck with both feet resting on the back of the deck getting ready to step onto the ground, his left foot slipped causing his left leg to hit
