A roof fall was discovered at approximately 10:55 AM on our #2 Belt. It measured approximately 6'H x 10'W x 30'L. All agencies have been notified.

EE was helping replace screens, they were putting a chute back in place when a bolt broke loose causing the chute to come loose. Chute struck EE on both legs causing him to fall.
Employee was bolting #4 entry. When he went to straighten roof bolt back in pot on bolter. He felt a pain in lower abdomen, and groin area.

While in process of using a auger drill, employee was drilling a hole for anchor pins, drill got hung in hole, causing drill's handle to spin, striking employee's left wrist, resulting in a scaphoid fracture.
WHILE IN THE PROCESS OF MOVING CONTINUOUS MINER CABLE FROM ROADWAY, EMPLOYEE STATED HE FELT SHARP PAIN IN HIS LOWER BACK. EMPLOYEE WAS SEEN BY LOCAL ER, WAS REMOVED FROM WORK AT THIS TIME.
Injured employee was removing his outer shirt when he struck his head against a roofbolt plate.
Employee stated he was walking from employee parking lot to shop when he tripped on water hose and fell. Employee did not report this to Safety Dept until 12-13-10. Is doing therapy on left calf muscle.

A battery power scoop started to travel through the check curtain, the bump from the scoop battery propelled the injured employee into the cutter boom of the continuous miner, resulting in a puncture wound to his left thigh.
While in process of installing permanent roof support, employee placed end of roof bolt into mine roof, while in the process of straightening bolt, employee's hand slipped, causing roof bolt plate to strike employee's right forearm, resulting in a laceration.
