Accident type, without injuries

A roof fall occurred on 5 section crosscut inby spad #11005 in the #5 entry. The fall measured 6' H. X 20' L. X 20' W. No one was injured. The fall was cribbed off, and dangered off. MSHA and State has been notified. The time it was found was 3:00 p.m.

A roof fall was discovered at Survey Station 24557 in the intake escape way #6 entry going to 002 section. This area was developed in October 2008. The roof fall measured 18 foot wide, 20 foot long and 10 foot high.

Roof fall was discovered adjacent to Spad 30417 by Mine Examiner while traveling return air course. The roof fall measured 20 foot wide, 160 foot long, and 10 foot wide. No injuries were reported as a result of this roof fall.

Roof fall was discovered inby Spad 30875 in the #3 entry by Mine Examiner during Preshift Exam. The roof fall measured 20 foot wide, 20 foot long, and 10 foot wide. No injuries were reported as a result of this roof fall.

A roof fall was discovered inby spad 23780 in the #4 entry a neutral, adjacent to 2A belt line, in the intersection. The roof fall measured approximately 19 feet wide, 20 feet long, and 10 feet high. No injuries were reported as a result of this fall.

Caught in, under or between a moving and a stationary object

Employee completed loading shuttle car from the #6 right, car left to return to feeder and traveled approximately 30 feet up a grade in 5 right. For unknown reason the shuttle car rolled backwards pinning him between shuttle car and the rib, resulting in fracture of the right leg and left ankle.

While in the process of dismounting supply motor employee felt pain in his left knee. He stated that he has a history of knee problems. On 12-23-11 an MRI was performed. On 1-6-12 he was seen by a specialist and was informed it was a new tear. This report was delayed to determine if this was a new or old injury.

Struck by falling object

While in the process of using a scoop to clean in an outby area, the scoop deck struck rib causing a piece of material to come loose and fall onto employee's back, resulting in contusions. An Xray was performed revealing no broken bones.

Over-exertion in lifting objects

On 10/05/2011 employee was carrying a guard to install on a continuous miner when he felt pain in his lower back. Employee continued to work, but the pain worsen at which time he was seen by his family doctor on 10/19/11 and removed from work.

On 10/3/2011 employee was installing permanent roof support, while straightening the bolt to place in the church of the pot, employee felt pain in right shoulder and arm. Employee continued to work, but the pain worsened at which time he was seen by his family doctor on 10/24/2011 and removed from work.

While checking the radiator of a bobcat loader employee's right ring and middle fingers came in contact with fan blades resulting laceration and fracture to both fingers.

Struck against a moving object

The trucks bed started sliding around to the left and he tried to correct it. His R/front tire struck the berm on his right side causing the bed of the truck to turn on around and his L/front tire struck the berm. This caused truck to turn striking the left berm below him with his left front tire causing the cab of the truck to overturn.

EMPLOYEE WAS RUNNING A CONTINUOUS MINER IN THE #5 HEADING ON 7 SECTION. HE WAS HANGING BACK A LINE CURTAIN WHEN A PIECE OF DRAW ROCK MEASURING 3FT BY 4 FT BY 1 INCH TO PAPER THIN FELL HITTING HIM ON RIGHT KNEE AREA.

Employee was riding on a rubber tire 4 wheeler. While traveling outby along 2D travelway, when a piece of falling material fell from the mine roof, striking employee on both legs, resulting in a fracture of the right femur and left knee cap.

Employee was in the process of shoveling belt when the shovel came in contact with the belt causing employee's left wrist to twist causing pain in his left arm & shoulder. Employee continued to work, but complained of pain in his left wrist, an MRI was performed, he was seen by a specialist on 10/17/2011, at which time he was removed from work with a tear of the fibro cartilage.

Employee was in the process of untangling a miner cable and water line when he tripped and twisted his left knee. He was taken to a local hospital; x-rays were performed with negative results. Injured employee returned to work and continued to work, but pain worsens. On 6-28-2011 a MRI was performed. On 7-11-11 the specialist removed him from work, to repair the ACL of his knee.

EE was drilling a hole. His glove was caught between the two pieces of finisher steel twisting right wrist. He continued to work until 5/18/11 when doctor took him off. No broken bones. The doctor told him to rest, and not use hand as treatment.

While extending a curtain during the mining cycle, employee hit the tip of his right index finger against a parked shuttle car causing pain. Employee finished his shift, on 4/29/2011 the pain continued to worsen; he was seen by a local physician who referred him to see a specialist on 5/2/2011. Specialist removed him from work to repair nerve damage on his finger.

Employee was bolting in #5 entry on 7 section. Drilling hole when steel broke striking employee on safety glasses causing a small cut under right eye. The cut require two sutures.

while in the process of removing a belt roller, the roller slipped causing employee's left little finger to be mashed against the belt rail, resulting in a laceration that required sutures to close.

Employee was in the process of installing a 10 foot roof bolt when a piece of falling material struck employee's right ring finger resulting in a fracture and laceration.

Cutting zip tie off self rescuer. Utility knife slip cutting right hand. The cut required 12 sutures to close wound.

He was cutting metal band off a pallet of roof jacks. The band snapped back hitting his right hand causing a laceration that required 4 sutures to close wound.

He was cutting a zip tie off leaky feeder line. The knife slipped cutting his left hand. 5 sutures were required to close wound.

Employee was walking toward roof bolter when his left arm was struck by a small piece of falling material, which caused a laceration that required sutures to close.

Making up roof bolt. A piece of falling material hit canopy of roof bolter. Slid off canopy hitting EE on left hand. It took 14 sutures to close wound.

Cutting zip tie that held up fire suppression hose. Metal whiz wheel cut his left finger causing 8 sutures to close wound.

While in the process of removing two pieces of drill steel from mine roof, top section of steel became lodged and separated from bottom section of steel. Employee placed his left hand on bottom, top section fell striking employee's left thumb resulting in a laceration.

Injured employee was working on a water pump when a piece of falling material struck his back. He shrugged the material off and it struck his left index finger causing a laceration.
