Accident type, without injuries

A roof fall was discovered by the mine examiner while traveling the airway of 6 right south mains # 6 cross cut inby the # 4 cut through. The estimated size of the roof fall is 6 ft high, 18 ft wide and 140 ft long.

A roof fall was discovered during the mine exam of the north east #5 seal, the estimate size of the roof fall is 6' high, 18' wide, 20' long. No injuries were reported as a result of the roof fall.

A roof fall approximately 30' wide x 35' long x 7' high fell around 2:30am at the #8 intersection in the 003 section return airway. This fall will be dangered off and will not be cleaned up.

A roof fall of approx 30'L X 20'W X 5.5'H was discovered at 8:30am by the Mine examiner. The falls location was 3 breaks from the bottom of the slope in the return.

A roof fall was found by a MSHA inspector and the Mine examiner, while traveling the return air way. The roof fall is an extension of a previous fall that was reported at an earlier date. The location is adjacent to 5B belt near spad #22048.

A roof fall occurred on the main track to 003-0 section. The estimated dimensions were 6' x 20' x 30'. No injuries were reported as a result of the roof fall. This fall is in the process of being cleaned.

While in the process of doing a preshift exam, a roof fall was discovered in number 3 entry. The estimated dimensions were 6' x 20' x 30'. No injuries were reported as a result of the roof fall.

At or around 3:00pm a roof fall occurred on the 004 section bleeder entry, just outby Spad No 23014. The fall measured 18 feet wide x 18 feet long x 8 feet high. No injuries were reported as a result of this fall of roof rock. The fall will not be cleaned and all entries leading to the roof fall have been timbered and danger signs installed.

A roof fall was discovered Tuesday morning 01/01/2008, 1 break outby spad 22161 on number 001 section. No injuries were reported as a result of this roof fall. The fall measured 18' x 18' x 6'. This area has been dangered off and will not be cleaned up.

Struck by falling object

Employee was changing bits while he was bolting when a rock fell from the roof and struck his right foot causing fractures to the metatarsal bones. *Amended - Due to infection, injured's toes were amputated. He remains off work.*

Injured employee was in the process of installing permanent roof supports. While removing the two piece drill steel from the mine roof, one of the two piece came apart and struck employee in the mouth, resulting in a laceration that required several sutures and dislodged some teeth.

Injured EE was walking around a continuous miner, when EE stepped on the miner cable that was lying on the mine floor, causing EE to loose his balance and straining his right knee.

Injured employee was loading belt structure into a scoop bucket when he injured his lower back.

Employee was helping load up some cable into a scoop bucket when he turned his body causing a sharp pain his right knee. He continued to work until 10/22/08.

Over-exertion in wielding or throwing objects

Injured employee was in the process of throwing a crib block, when he experienced pain in his lower back, Injured employee was seen by the local ER physician and taken off work.

Struck against stationary object

Injured employee had finished drilling a test hole in the mine roof. While in the process of placing his drill steel in the tray compartment his hard hat struck the bottom side of the canopy, resulting in a hairline fracture to C5.

Fall to the walkway or working surface

Employee was in the process of hanging miner cable when his feet got tangled up in the cable causing him to fall to the mine floor. When he fell, he tried to catch himself. This resulted in a bruised right wrist. The employee continued to work and completed his shift. EE did not return for next shift.

EE was recovering pinner cable from under some debris. After recovering the pinner cable, he stood up and felt a sharp pain in his lower back.

Injured EE was in the process of removing a cover lid from a continuous miner to place on mine floor, when EE stepped on some loose bottom, resulting in EE spraining his left ankle. Injured EE continued to work until 12/22/08, when his family doctor removed him from work.

Fall from machine

Injured employee was operating a 10 ton locomotive on the outby end of a supply train. Employee became panicked while descending a slight grade and jumped from the moving equipment. When landing, the employee suffered contusions.

Injured was a passenger on a johnson rubber tire vehicle that was in the process of parking next to a power center when his right knee struck an electrical plug. This resulted in a laceration which required medical treatment. He was given a full release from the ER physician to return to work on 7/1/08. He in return visited his family doctor and was removed from work.

Employee claims that while carrying his lunch bucket and a bucket of miner bits from the elevator doors to the 6 ton battery station. Employee stated that he felt pain in his back, employee visited the local hospital and was seen by the ER and was removed from work at this time. 6/24/08.

Injured employee was in the process of installing permanent roof support, he had put together the roof plate and resin rod to be installed, pitched them toward the drill boom, as he was walking toward the drill boom, a piece of draw rock fell from the outside roof bolt toward the rib, striking his right shoulder and right foot, resulting in fracture to his foot.

Employee was repairing a conveyor chain when he slipped and fell to the mine floor. He was transported to a local emergency room where x-rays revealed a fracture to the spineous process of the L 3 vertebrae.

EE was loading supplies with a front-end loader. As he was moving forward the front wheel came off of the machine. He continued to work the reainder of his shift. Later that day he visited a local hospital's emergency room where the attending dr diagnosed his condition as degenerative disc diease and contusions. Mgts feels due to dr' opiona. this is best classified as occ illness.

EE was riding a rubber-tire mantrip, when the man trip hit a hole in the roadway. This resulted in the EE's left foot to come in contact with the front tire of the machine, causing a contusion to his left ankle.

Employee was opening an airlock door at the bottom of the elevator when he felt a pain in his neck. He continued to work approximately 4 hours until his neck got stiff.

Injured EE was operating a battery powered 6 ton man trip that was approaching 5A track switch, when a second battery powered 6 ton man trip struck the operator compartment of the injured EE's man trip, resulting in fractures to his left foot.

While installing a roof bolt, the bolt bent. The EE placed his hand on the bolt and engaged the rotation level on the roof bolt machine. The roof bolt spun around, striking the EE on the right thumb.

While installing a roof bolt, a piece of draw rock fell, striking EE on the right hand, resulting in a fracture to his right little finger.

EE was installing perminant roof support, when a piece of draw rock fell, striking him on the left foot. EE finished the shift, however later on that evening, he went to a local ER where X-rays revieled he had a fractur to the bone directly behind his big toe.

Injured was in the process of lifting a piece of belt structure, to remove from a tail piece when he felt a sharp pain in his left shoulder. He returned to work his next shift. On 10/11/08 he visited his family dr and was removed from work.

Injured was working on the cable reel of a shuttle car. When the cable reel freed up the cable caught his right leg. This caused him to fall and injured his right knee.

Injured employee was removing a piece of draw rock from the top of a continuous miner. His right hand slipped, causing a laceration to employee's right thumb, which required sutures to close.

Employee was in the process of setting timber when a piece of draw rock fell striking the employee's cap light on the front part of the hard hat causing the bill of the hat to strike employee's nose resulting in a fracture to injured employee's nose.

Injured was in the process of hanging a shuttle car anchor to the mine roof, when a small piece of coal fell into his right eye. He visited the local hospital and was seen by the emergency room physican to have it removed.

Injured employee was in the process of cleaning the shop. While picking up a grease bucket lid, employee received a laceration to his right hand. The laceraation required 3 sutures to close.

Injured employee was installing permanent roof supports. While in the process of removing the drill steel from the mine roof the drill steel fell, striking injured employee's left eyebrow. Resulted in a laceration that required 3 sutures to close.

EE was installing permanent roof support when the roof bolt he was installing bent. In the process of removing the bent roof bolt, he accidently activated the rotation device on the roof bolting machine causing the bent bolt to rotate. The roof bolt plate struck him on the left forearm causing a laceration that required several stitches to close.

Two EE's were in the process of tearing dwon a brattice. While removing a cap wedge, the hammer struck a roof plate, causing a small piece of metal to stirke the injured EE in the left eye. Injured EE was seen by the local ER physician and received medical attention. The physician gave him a full release to return to work.

Empoyee was in the process of making a permanent splice in a trailing cable, the utitily knife he was using in the process came into contact with his finger, resulting in a laceration that required sutures to close.

Injured employee was standing on top of a parked scoop hanging a ventilation curtain. While attaching the ventilation curtain to a roof plate, he lost his balance and reached for another roof plate to keep from falling. The roof plate he grabbed had a sharp edge causing employee to receive a laceration to his left thumb that required several sutures to close.

While operating a shuttle car behind the continuous miner, a small object ejected from the scrubber, striking injured employee in the right eye.

Injured EE was drilling a hole for a roofbolt, when a piece of draw rock fell, striking his right hand and fractured his ring finger.

EE was walking around a parked scoop, when his right ear struck a roof plate, resulting in a laceration that required 6 sutures.

Injured employee was using a utility knife to cut the straps on a large bag of rock dust. While in the process of cutting the strap, the knife struck injured employee's right forearm, resulting in laceration that required 15 sutures to close.

Injured employee was operating Johnson ride when the front tire struck a piece of Omega block on the mine floor. This caused the steering wheel to turn suddenly and the steering knob hit his right hand resulting in a fracture.

Struck by flying object

Injured employee was assisting another employee who was using a pole axe to cut a belt pin when a piece of metal struck injured employee's left eye. Injured employee was seen by the ER physician and then was recommended to an eye specialist for further treatment.

Injured EE was lifting belt structure with a chain ratchet; hook that supported the chain broke, causing the structure to strike the EE in the left forearm, resulting in a hairline fracture.

Rubbed or abraded

Injured employee had been using a grinder to down size a pin for a shuttle car. He removed his safety glasses and was in the process of wiping his face when a small piece of metal got into his right eye. Injured employee was seen by a local eye where a small piece of metal was removed.

Caught in, under or between a moving and a stationary object

EE was helping the continuous miner operator turn a break. When the ripper head was lowered, the bumper of the miner set down on EE's left foot. X-rays taken indicated EE received fractures to 3 of his left toes. EE returned to work on his next scheduled shift.

Injured ee was in the process of putting fish plates on two joints of track rail. When another ee moved the rail that was been installed causing the rail to turn over resulting in a fracture to injured ee left thumb.

Employee was using a 10 ton lifting jack to lift a derailed track motor. While in the process of applying pressure to the jack bar, employee's hands slipped, resulting in the jack bar striking injured employee's lower jaw.

Injured employee was in the process of changing battery on a battery powered scoop. When the main breaker knocked, injured employee placed his left hand on the reset handle and was applying pressure when his left hand slipped and struck the cover, resulting in a small fracture to the ring finger.

While taking down a miner cable that was supported from the mine roof by a metal cable hanger, EE was struck by the hook end of the hanger between the right index finger and middle finger resulting in a laceration requiring three stitches to close.

While loading belt rollers into a scoop bucket, EE got his right middle finger caught between two rollers. He worked until 2/11/08. At this time he made a visit to family physician where x-rays show he has a fracture to the tip of his right middle finger.

EE was operating a tired mantrip. EE drove over a piece of draw rock causing the steering wheel to be jerked. This jerking of the wheel caused it to spin around striking EE on the right hand resulting in a fracture to his right hand.

Injured employee was in the process of installing a curtain board to the mine roof. He had placed his left hand around the wrench when he turned the rotation on to tighten the roof bolt. His glove was pinched between the wrench and drill pot causing fractures to his middle, ring and little fingers.

EE was walking to his shuttle car. While reaching down to remove a piece of curtain out of the roadway, he felt pain in his left knee. The EE was taken to a local emergency room where his left knee was examined and x-rays taken. No abnomalities were noted. Employee was told to follow up with his family Dr. on Monday, March 31, 2008. At his time he was taken off work.
