Accident type, without injuries

A roof fall was discovered by a belt examiner at cross cut 38 of the No.2 conveyor belt. The area was developed sometime in 1982. The roof fall measured approximately 20 feet wide, 25 feet long, and 8 feet high. No injury was reported due to this roof fall.

A non-injury accident occurred when the elevator became inoperative due to a malfunction on the main control cable at the bottom of the elevator. The main control cable has been repaired and inspected.

While hauling supplies along #4 travelway a large bag of rockdust contacted a steel beam that had been set in the area for additional roof support. This dislodged the beam and allowed a piece of rock measuring approximately 9 foot wide, 13 foot long, and 4 foot thick to fall, preventing travel through area.

A roof fall was discovered outby spad 33190 during preshift exam. The roof fall meas. 19' Wide, 30' Long, and 9-10' in Height.

Rock Fall on Break 5 on 2 belt. No one was injured. Measured 20 by 20 by 6 foot high. Msha, and State inspectors were called. Both came to mines.

A non-injury accident occurred on the secondary escapeway hoist. Hoist lost program and was not operating, mine was evacuated. Hoist program personnel arrived and reprogram and inspected hoist.

Struck by falling object

While in the process of removing the two piece drill steel from mine roof, the top piece separated from the bottom piece. Bottom piece fell striking injured employee on his pinky finger resulting in a amputation to the tip of finger.

Employee was walking around the front of 1C tail piece, when he stepped into some uneven bottom causing him to slip and twist his left ankle.

Struck against stationary object

Employee was advancing the Fletcher roof bolter forward using the inch tram controls when his left foot stepped on some loose material that was on the mine floor causing him to twist his ankle.

Employee was in the process of installing the outside roof bolt in the last row, he had just put the two pieces of drill steel together and was in the process of starting to drill while applying up pressure when the collar broke resulting in the bottom piece striking him on the left side of his face.

Over-exertion in pulling or pushing objects

Due to the coal seam height, the employee was bending a 6' roof bolt preparing it for installation when he felt a sharp pain in his lower back.

Over-exertion in lifting objects

Employee was operating a battery power scoop lapping up conveyor belt, he exit the scoop to help lift one of the belt loops when he felt a sharp pain in his lower back. Employee was seen by a local ER physician and was removed from work.

Struck by falling object

Injured employee had backed the continuous miner into the intersection, shut miner off and was in the process of moving miner cable away from the side of the pan at the back of the ripper head, when a piece of material slid off the continuous miner striking his left foot resulting in a fracture.

Injured employee was in the process of installing permanent roof support. He had placed the end of roof bolt into the mine roof and was in the process of straightening the bolt when he felt a pull in his lower back. He continued to work until 10/30/12 when he was seen by a local physician and removed from work.

Injured employee was in the process of installing permanent roof support. He had completed the drilling process and was reaching for the bolt that was laying on the mine floor next to drill pot when a piece of material fell striking his left index finger resulting in a laceration and fracture.

Caught in, under or between a moving and a stationary object

EE parked the shuttle car that he was operating, as he was using a pick type hammer to pull come loose material from the rib, his right forearm was caught between the material that he was scaling and the shuttle car. EE continue to work until 10/15/2012 when he was seen by a local physician and placed off work to see a specialist.

Skill saw kicked back while cutting steaks for concrete forms cutting right thumb of employee requiring surgery on 8/10/12.

Employee was cleaning shop when he stepped over a crib block his knee buckled.

Employee was in the process of cleaning a tail piece. As he was shoveling under the tail piece the bottom belt came in contact with the shovel, jerking the shovel causing pain in his right arm. He ceased work and was taken to a local hospital where X-Rays were taken. He was taken off work until a follow up on 8-2-2012.

Employee had a timber that was knocked. He got it out of roadway, and laid it on the rib. His buggy cable pulled it back out in roadway, and when he came back from miner with a load of coal. He ran over timber hitting hard hat on buggy canopy hurting his neck.

He was hanging back miner cable, and water line when he felt pain in lower back area.

Employee had finished installing a roll of roof bolts and had walked back to the dust box, as he was bending and reaching for the straps to open dust box a piece of falling material struck his right forearm causing a laceration. Physician removed him from work due to risk of infection.

While in the process of drilling test hole employee placed his left hand on glue tray, a piece of falling material struck the tip of his left ring finger resulting in a avulsion to the finger tip that required the finger nail to be removed. A physician placed him off work due to risk of infection.

EE was on his knees bolting, he had a piece of drill steel stuck in the roof, he lost his balance and caught himself by putting his hand on the top of the drill pot, the drill steel dislodged from the roof and struck his middle finger on his left hand resulting in a broken finger.

While in the process of banding roof bolts on a supply car, strap broke, striking employee on his left brow, resulting in a laceration that required glue to close.

While in the process of applying fire extinguishers and rock dust to the high voltage compartment of the 4E power center that was smoking, employee started to hyperventilate and experienced a loss of consciousness. This incident is being reported only to abate a MSHA citation.

EE was in the process of reassembling the headlight on a battery powered 4 wheeler, his hand slipped striking a sharp piece of metal, causing a laceration that required sutures to close.

Employee was in the process of making a belt splice using a Flexco hammer, when a sliver of metal struck his right index finger, resulting in a laceration that required sutures to close.

Employee was unloading block off a supply car at the end of the track when some blocks fell off the pallet onto the mine floor. As employee reached to pickup the blocks to re stack them one of the block fell from the pallet striking his right hand causing a puncture between the thumb and his index finger that required sutures to close.

Employee was in the process of installing permanent roof support. He was position next to the tram controls when a piece of falling material struck his right foot resulting in a hairline fracture to his little toe.

While installing permanent roof support EE tripped and fell on a roof bolt plate that was laying on the mine floor, resulting in a laceration to his left leg that required sutures to close.

While in the process of using a utility knife to cut belt, knife slipped resulting in a laceration to employee's right leg.

EE and three other employees was lifting a 15 foot beam to load onto a supply car, beam slipped causing it to mash his left middle and pinky fingers.(On 05/18/2012 surgery was performed on EE's left middle finger, he returned to work 05/19/2012)

Employee was unloading jackleg from shaft for repair, drill folded back pinching middle finger right hand. 8 stitches
