Struck against stationary object

Employee was checking the hardness of a sheet of plate steel. While making this check, employee's hand came in contact with the rough edge on a piece of plate steel adjacent to the piece being checked.

Struck by falling object

Employee was operating forklift in the plant warehouse. While backing up, the forklift hit a box, knocking it over. Employee tried to stop the box from tipping over with his right foot.

Fall to the walkway or working surface

THE EMPLOYEE WAS MOVING A SOUND SCREEN AND DESCRIBED A MOMENT OF INATTENTION. AT THAT TIME HE ALLEGES HE HOOKED HIS RIGHT FOOT UNDER THE SCREEN FRAME AND TRIPPED, DROPPING HIS RIGHT KNEE TO THE GROUND.
