Employee was throwing a bag of trash into the dumpster.
Climbing extension ladder to get platform on front of crusher to check main belt take-up adjustment. Slipped and/or lost footing and fell about 8' on back.
Cleaning off a table in the raw mill, employee stepped over wheel, leg got stuck and twisted ankle.
Disposing excess raw material in appropriate pile. Tripped over unseen rock and fell on left side, elbow, arm and shoulder.
Swinging sledge hammer to open slide gate.
Employee did the line change on the 6"" line to set up in cement Silo, then came down from Silo to inspect Kiln grease system and proceeded to clean additive system shoveling material and pain got worse. EE attempted to sit in the 320 loader, but the pain intensified. EE immediately reported it to the control room to report it to the supervisor.
Over the course of over three to four months and with increasing frequent use of grinding bowls and auto-sampler buckets, tendinitis set in to left thumb and right forearm.
