Fall down stairs

Employee was on equipment inspection prior to start up of crusher. Slipped/tripped on the steps of the primary belt and broke right ankle. Cleanup was in progress at the time of the accident to clear the light dust and sparse rock.

Contact with hot objects or substances

Build up from the preheater dropped onto the feed shelf, which caused hot kiln dust to spill from the feed end air seal. This spillage then fell 26 feet to the ground floor.
