Struck by falling object

Employee entered kiln to help scale during kiln shutdown. A large piece of coating came loose and rolled down pile and hit his foot.

Over-exertion in pulling or pushing objects

This was reported as a first aid on 11/12/12 but employee did not seek medical attention until 12/5/12. Employee was pulling electrical cable in a tray when his retractable engaged causing him to twist his back.

Fall to lower level, (Not Elsewhere Classified)

The Employee, was working at height and fell approximately 9 feet to the surface below when the weld of tie off point failed. Employee sustained broken ribs and bruising.

Caught in, under or between running or meshing objects

Employee was grinding the rough edges off of a 4"" x 6"" x 1/8 plate using a bench grinder. He tilted the plate at an angle and the wheel sucked the plate into the gap between the guard and grinding wheel, lodging his finger between the wheel and plate.

Struck by... (Not Elsewhere Classified)

Employee was cutting with a torch and when completed he flipped up his shield and dust rolled off of shield behind his safety glasses into his right eye.
