Accident type, without injuries

Their was not any injury(s) or equipment damaged nor did it impede ventilation. The roof broke above the bolting horizion. Location 29 South @ 17 East. Size of fall is approximately 50ft x 50ft x 14ft. This area was blocked off to all mine personnel since 5/24/04 anticipating this fall.

Accident type, without injuries

Their was not any injury(s) or equipment damaged nor did it impede ventilation. The roof broke above the bolting horizion. Location 34 North @ 10 East. Size of the fall is approximately 50ft X 90ft X 15ft. This area was blocked off to all mine personnel for the last 3 days anticipating this fall.

Slipped on tracks of scaler and hurt lower back

Oct 13,2005 employee was walking across shop floor and felt something snap in his left knee. He was not able to see a doctor unitl 10/17/2005.

Over-exertion in lifting objects

Employee was stacking 50lb bags and felt a pain in his left arm.

Struck by falling object

While scaling the face of 14E - 31N, a piece of rock came off of the rib and hit the employee on the right ankle.

Employee was taking a drill steel apart with a pipe wrench and the wrench slipped off striking the employee in the face.

Contact with hot objects or substances

Oxygen hose failed below the fitting near the torch-end thus causing a fire and igniting the employee's glove on fire. The burning glove caused second and third degree burns on the right hand of the employee.

Over-exertion in lifting objects

Employee was lifting and felt a pain in his groin area. This accident was reported to MSHA in Dec. of 04 and the employee was off for 42 days. This is a reoccurring injury.

Struck by falling object

Employee was helping close the hood on #4 kiln with the south trolly when the coal pipe hit the top handrail and the handrail broke off and fell about 15ft striking the employee on the right arm.

Over-exertion in lifting objects

Employee was working and complained of lower back pain. The original accident report was filed on 8/11/03.

Over-exertion in lifting objects

Moving cable and twisted thumb. This accident occured on March 4, 2005 and the employee did not seek medical attention until Sept 28, 2005.

Fall to the walkway or working surface

Slipped and fell on left leg, twisting hip and ankle. Employee did not seek medical attention until 11/03/05

Employee was lifting 8"" concrete masonry unit, twisted and strianed his lower back

Caught in, under or between a moving and a stationary object

THE WORKER WAS MOVING SCRAP METAL AROUND AS HE WAS MOVING A PIECE AND A PIECE BESIDE IT FELL OVER AND PINNED HIS LEFT HAND BETWEEN THE TWO PIECES OF SCRAP STEEL. HIS 2nd & 3rd DIGIT FINGERS WERE BROKEN.

Fall from ladders

Was coming down a ladder and slipped and fell on left side.

EMPLOYEE LOSS HIS GRIP ON GRINDING TOOL AND WHEEL HIT HIS GLOVE AND CUT HIS THUMB

Fall onto or against objects

Employee was pushing a wheelbarrow and his foot slipped causing him to hit his chin on the wheelbarrow and cutting his chin.

Struck by flying object

Water line blowed apart on the pressure washer, blowing water into left eye under safty glasses.

Struck by flying object

Employee was cleaning dust collector lines and got dust in his eye. Was pounding with a hammer.

Employee was pulling on an air hose and the valve got turned on and it whipped and struck him by his left eye.

EE was pushing crate off of pallet jack and slipped and fell on left knee.
