Employee bent over to pick up a switch and could not stand back up.
The haul truck the employee was driving overturned and came to a rest on the passenger side
Employee was unloading trailer with lime and the agitator line came off end of hose and hit employee in the face-head.
Employee was attempting to open a lid to our pneumatic tank. The lid was frozen shut. The employee pulled the lid to open it. This caused a hernia to the right side groin area.
