Graymont (PA) Inc.Mining Incidents in 2013

The employee slipped on ice and fell twisting his leg.

Employee bent over to pick up a switch and could not stand back up.

The haul truck the employee was driving overturned and came to a rest on the passenger side

Employee was unloading trailer with lime and the agitator line came off end of hose and hit employee in the face-head.

Employee was attempting to open a lid to our pneumatic tank. The lid was frozen shut. The employee pulled the lid to open it. This caused a hernia to the right side groin area.

Employee was operating a haul truck traveling down rough roadway in mine. The back end of the haul truck bounced a couple times from hitting uneven road which caused injury to the lower back.

Employee was decending from ladder and his foot slid down bottom two rungs and twisted his knee when his foot hit the floor

Was cutting zip tie from hose reel to be installed and cutting blade slipped and cut right index finger requiring stitches.

Employee was tightening a fitting on water pump of roof bolter when wrench slipped hitting left thumb

Employee started noticing symptoms of carpal tunnel and scheduled an appointment with physician on 1/31/13 to get it checked. Symptoms started about 3 months prior to accident date above. Physician scheduled surgery for 2/19/13.
