Accident type, without injuries

Roof fall in North East Section of the mine. Nobody was injured. Damage to brattice did occur. MSHA and DEP are notified. Orders issues by MSHA and State PADEP.

Drive belts on Metso vibratory screen were improperly tensioned, which either caused or was caused by the screen being overloaded. This caused the belts to slip and catch on fire, which ignited the screen dust cover. Damage was limited to drive belts and dust cover.

Discovered 7:47 am, intersection of 35 E 42 S.

Over-exertion in pulling or pushing objects

Employee pulled stomach muscle pushing/sliding the rack to load a trailer. Possible hernia.

Struck against stationary object

Tramming Fletcher Scaler and the tram motor went out causing it to free wheel downhill. Miner applied manual break. Scaler traveled 10-20 ft. and hit rib. On impact, miners leg slipped off break and hit the bracket holding the fire suppression system in cab causing a laceration.

Rubbed or abraded

Employee was washing a small glass cup used in an instrument to test lime quality. The bottom of the cup cracked and fell out, employee cut hand on sharp edge.

Miner complained of chest pains while driving truck through the portal on way to surface office. Forman contacted and ambulance called for care. Miner was transported to hospital. Returned to work next day OK, pains believed to be caused by acid reflux, pollen allergies and high blood pressure.
