Caught in, under or between a moving and a stationary object

EE was attempting to remove a broken drill steel when the chain slipped pinching their right thumb

Employee went to pull the hinged ladder down to inspect the belt and after releasing the ladder while holding it with their right hand, they released the ladder which smashed their left pinky finger between the ladder and corner of the catwalk grating. Employee was wearing A4 cut resistant and impact resistant gloves at the time of incident.

EE was inside the primary crusher cleaning out large rocks. EE tweaked back while moving around. EE sustained a strain to the back and pelvis. Medical treatment was sought after.

Miner attempted to remove a shovel that was frozen in the back (bed) of a pick-up truck.

Fall to the walkway or working surface

A Dragline Operator has pain in right shoulder after they tripped and fell on the same level. The miner and coworkers were preparing to install a new drag cable. As the miner was moving backwards with cable in hand they tripped over a dirt clod lying in the field and fell on their back and elbow and jammed an arm. An injection for pain now makes this reportable.

Struck by falling object

A miner sustained a contusion and laceration to the area below nose and above upper lip when EE was struck by a falling object. The miner was adjusting the v-belts on the crusher and while positioned under the motor loosening the mounting bolts to move the motor, a spacer fell about 6 to 8 inches and struck the miner causing the injury.

Contact with hot objects or substances

A Loader Operator sustained burns to his hands while helping a coworker make a guard for the C15 tail pulley. The coworker was using a torch to trim the guard and the injured observed more hose was going to be needed to complete the cut. While pulling the oxy/fuel hose into position it ignited in his hand resulting in the burns.

Over-exertion (Not Elsewhere Classified)

A blaster strained his left knee while mounting a Komatsu PL400 Excavator. The injured employee placed his right foot on the step and his left on the track. When he shifted his weight to his left foot, his foot went between the pads and he twisted his left knee. The injured miner did not request medical treatment until September 8, 2011.

Over-exertion in lifting objects

A Dozer Operator is suffering from low back pain after picking up a tooth for the Dragline bucket. The injured miner was helping change the teeth on the Marion 7400 Dragline when the incident occurred. He had handled several teeth then while picking up the fifth tooth he felt pain in his lower back. He did not request medical treatment until June 29, 2011.

The injured miner was getting ready to leave work for the day when he realized he had left his lunch box in the garage. Upon entering the garage to retrieve his lunch box his left leg went out from under him resulting in the injury. Additional medical testing, positive MRI now makes this incident reportable.

Employee reported upper back strain. Told supervisor that he hurt it swinging a mall when repairing a front end loader. Several days later he said it may have been hurt when working with a porta power unit. Was unsure what activity caused the pain or when he hurt his back.

Employee was dismounting from skid steer. He sustained a strain when placing weight on his foot.

Accident type, without injuries

Excessive rainfall during the previous day has allowed enough water to collect in the pit such that normal operations in the pit have been stopped. No personnel or equipment were affected as a result of this accumulation of water in the pit.

Fall onto or against objects

A mechanic suffered a contusion to his right shin while attempting to access the bed of a service truck. The incident occurred when his foot slipped from the bumper causing him to strike his shin against the bumper. He is now being treated for an infection as a result of the contusion to his right shin.

Over-exertion in lifting objects

A Truck Driver injured his right arm while assisting coworkers with a screen change. The injured employee was positioned below his coworkers and was attempting to lift a piece of screen cloth when the injury occurred. Additional medical tests (MRI) have been conducted and on May 23, 2008 it has been determined the injured has a torn biceps which will require surgery to repair.

The employee was tasked with performing the monthly fire extinguisher inspection. The employee was in a cab of a front end loader and dropped the extinguisher on the cab floor. As he retrieved the extinguisher he activated it and the dry chemical hit the cab floor and sprayed him in the face.

The injured was climbing a ladder at the plant and failed to use three point contact and fell approximately five feet to the ground.

The injured and a co-worker were cleaning a hopper when a chunk became loose and struck the injured on the right leg.

An excessive amount of snow, sleet, and rainfall Wednesday, 1/5, and into Thursday, 1/6, caused flooding of the quarry at this location. Weather permitting, normal operation is expected to resume Friday, January 7, 2005.

An excessive amount of rainfall Tuesday, December 7, 2004, caused flooding of the quarry at this location. Weather permitting, normal operation is expected to resume Wednesday afternoon, 12/08/04.

Fall onto or against objects

On April 9, 2004, the injured was dismounting the hydraulic shovel when he bumped his knee. He was cleared to return to work that day. After further medical attention, this injury is now recordable.
