Struck by flying object

Partical in eye, resulting in a corneal abrasion.

Over-exertion in pulling or pushing objects

Employee was putting fuel hose into fuel truck to fill up truck. Employee felt a pull in his side.

Fall from machine

GOT OUT OF CAB OF WATER TRUCK TO MAKE SURE IT WAS LINED UP. STEPPED ON LEFT SIDE OF CAB AND LOST HIS FOOTING, FELL APPROXIMATELY 72"" TO THE GROUND.

Fall onto or against objects

Employee was opening the engine door to check the oil. EE backed up & fell off the loader hitting his right wrist on the manlift parked next to the loader.
