Fall from machine

Employee was assisting on blast crew as a driver & laborer. He was attempting to exit the rear of a straight van truck. He was using the rear door and door frame as hand holds & was reaching with foot to use ICC bumper as a step when his foot slipped. He fell to the ground injuring his right knee dan right shoulder.

Over-exertion in lifting objects

EE was lifting an empty oxygen bottle off of the welder truck when he felt pain in his right shoulder. Initially this was reported as first aid however after testing was performed it was determined that surgery was required. EE surgery is scheduled for 12/15/2010.

Struck against stationary object

EE was pushing a wheel barrel and was too close to an Ibeam. He bumped into it and hit his hand.

Over-exertion in lifting objects

Employee reported on 3-1-2010 he felt pain in right groin while assisting in lifting a steel plate..he did not request medical treament at the time of report. 12 days later he did seek medical treament and was diagnosed with a hernia and scheduled for surgery on 3-26-2010 and estimated to be off work for 4 weeks.

Fall from machine

Employee was coming down the ladder on a 988 Cat loader. He had fueled the machine up and had the nozzle in his hand. He had three points of contact & he thought he was on the bottom step and realized he wsn't. He missed the bottom step and hurt his right knee. He said it didn't really hurt at the time but the next day it was swollen up.

Contact with hot objects or substances

After cutting bushing off of shaft of speed reducer, he leaned over to look at the work, and while holding torch in left hand inadvertently moved torch over his lower right arm causing burn.

Caught in, under or between a moving and a stationary object

While installing speed reducer on head pulley conveyor 26, EE pushed the speed reducer onto shaft. Injured had his hand on back side wiggling the reducer for it to slide onto shaft. When it shot on, EE's right hand fifth finger got smashed between the speed reducer and a piece of angle.

Caught in, under or between a moving and a stationary object

Employee was doing maintenance on a crusher and caught his finger between bar and side of crusher
