Caught in, under or between a moving and a stationary object

EE was guiding inside part of breaker into case of main breaker when it pushed forward pinching finger between both parts cutting end of little finger almost off.

Employee stepped out on catwalk of loader to clean windows when he felt something pull in his left knee area.

Trip over door jamb going into building.

Employee was exiting his personal vehicle, standing by car door, reached in to grab lunch box from car seat and his right knee popped. resulting in a right knee acl tear.

He was walking across driveway toward the Lab Building after putting trash in dumpster. His right ankle rolled out from under him causing him to fall. He fractured and partially dislocated his right ankle. The driveway is level and covered with 1B stone.

Pocket knife in hand, cutting tie strap, knife slipped cutting left hand between thumb and index finger.

Caught in, under or between a moving and a stationary object

EE was trying to unplug the chute on the jaw crusher when a plate fell down in the chute and he was using a prybar to try to pry the plate loose when his finger got caught between the bar and the chute. This pinched his middle finger on his left hand. His finger is fractured.

Over-exertion in lifting objects

EMPLOYEE BENT OVER AND PICKED A 5 GALLON SPRING WATER BOTTLE (FULL)WITH ONE HAND AND SLIGHTLY PULLED LOWER BACK MUSCLE

Struck by flying object

EE was using an air tool with a flapper wheel installed on it. He was working on a hoist cylinder for a haul truck. He was cleaning the packing seats to replace the packing. While cleaning the seat a particle of foreign material got in his left eye. He was wearing safety glasses, dust mask and ear muffs but admitted he should have worn a face shield or goggles.

EE was working on front suspensions of pickup truck. He was holding a breaker bar with socket on a nut while other EE was using a 1"" drive air powered impact wrench to loosen nut when the breaker bar spun pinching ring finger between shock and breaker bar.

Fall onto or against objects

Employee was climbing down from conveyor head pulley chute and slipped off of a shaft for the flop gate. His leg scraped against the shaft as he fell and scraped his leg causing lacerations.

Struck against stationary object

Employee was walking around the screen when he bumped his elbow on a piece of angle iron bruising it.

Over-exertion in pulling or pushing objects

Employee is still complaining of pain in left thumb.....Went to the doctors again and this time put him on modified duty.

Pain in the left thumb. Employee went to his own family doctor and then referred to the company's doctor. Doctor says it is trigger thumb from repeating motion by pulling on the handle to loader.
