Over-exertion in lifting objects

Employee was cleaning between Screen 6 & 7 when employee attempted to move a metal plate on the ground. Employee felt a pop in their groin. Employee was taken to clinic for evaluation.

Struck by flying object

Employee was welding on a metal strip which was caught in a feed chute of a crusher. The metal was cut free and sprung out hitting the employee and knocking the employee backwards.

Caught in, under or between a moving and a stationary object

Employee attempted to remove a stuck safety chain on a crane block. The workers finger got caught in the block and resulted in a injury to EE's left hand.

Struck by falling object

Employee was struck by a falling rock from the level above were EE was working. The employee received a cut to the back of EE's head which required 2 stitches to close.
