Over-exertion (Not Elsewhere Classified)

Employee was leaning over frame to fix an O-ring on bottom of hoist pump and had to stretch to reach the lower bolts. He heard a pop in lower rib, left side and felt a sharp pain.

Fall onto or against objects

While placing a bolt in the hole miner stumbled and wrist contacted a bolt plate resulting in a laceration.

Struck by falling object

Employee slipped while holding Jack-Leg drill and the machine fell and smashed his left index finger against the rib.

Miner was pulling a stuck jackleg steel out of the rib. When steel came loose the miner fell backwards striking the edge of a bolt plate with his right elbow, resulting in a laceration.

Caught in, under or between a moving and a stationary object

Mechanic was pulling out the boom safety lock pin (pin was stuck) on an LH204 Loader, after repairing a hydraulic hose. He pinched his right middle finger between the pin and a headlight bracket on the boom.

Fall onto or against objects

Miner slipped and fell while turning around. Extended his left arm to break his fall and his hand struck a sharp rock on the low rib, resulting in a laceration to the outside edge of his left hand.
