On November 23, 2016, at the start of day shift an underground miner entered the 2-5551 S to retrieve a can of paint. As EE was doing the work place inspection while entering this area, EE noticed a fall of ground and called the supervisor.

HCN gas alarm activated in acacia reactor in mill building; HCN gas detected outside acacia reactor room with hand-held gas meter. Mill building evacuated and ventilated. Lime water applied to overflow from acacia brought pH up and eliminated HCN generation.
Employee was manipulating control arm from the right to left position on the Stinger Longhole drill. This action resulted in a pulled muscle in right shoulder.
Employee was moving a size 200-oxygen bottle to the cascade pump at the mine rescue station. When EE lifted the oxygen bottle, EE felt a pain in EE's left arm above the elbow. Diagnosed with partially torn tendon.
Employee was working on fixing the oil dip stick that attaches to the oil pan when the engine cover fell down and struck the employee on the right hand and caused a laceration to the palm portion under the right thumb. This laceration received stitches.
While adding vent bag dust blew into face and eyes, irritating right eye.
