Accident type, without injuries

A contract production driller moved drill to continue drilling a series of uppers at 1-5204. Driller noticed some loose material starting to dribble from right rib at far end of drill. Driller notified the shift supervisor & was advised to stay out of heading until further notice. The shifter arrived & noted that the right rib had failed beyond the drill.

Operating a scraper in a fill, tire went into depression in ground, air ride seat bottomed out jarring operator.

While pressing the cab mounts pin on an R1600 the bar they were using with the press slipped out while under pressure. Bar slipped out striking EE in left side of mouth. Causing a split in upper and lower lip. 2 Stitches required in upper lip.

Fall to the walkway or working surface

Contractor employee was walking on site and had to cross the 2-step bridge over cabling on the ground to eliminate a trip hazard. The area was icy, covered by snow. Employee slipped coming into contact with the sharp edge of the step tearing EE's leather work glove and causing a laceration to the left thumb requiring sutures to close.
