Accident type, without injuries

Knight Hawk Coal LLC, Royal Falcon Mine found a roof fall in the #1 entry of the Main West at the +430 cross cut in the return. The fall did not obstruct any escapeways, air courses, and was outby the tailpiece of the section. The fall was called in to MSHA and we have done what our district asked of us. No employees or equipment were involved in the fall.

Knight Hawk Coal LLC, Royal Falcon Mine has experienced an unintentional roof fall. The fall happened in the first set of return rooms of the 1st east panel at the +288 cross cut in the #3 entry. No persons or equipment were damaged. MSHA was contacted at 2:19. PM. The area fell at 2:15 p.m. and is now dangered off and cribed off.

An unintentional roof fall occurred in the Main East Entries (001-0) off the Main North Entries in #2 entry at #13 cross cut +1140'. The fall was approximately 8' wide by 20' long. MSHA was notified within the prescribed time frame.

An unintentional roof fall occurred on the 1st North Panel (#1 Unit 001) off the Main East in the #6 entry at 8+40 cross cut. The fall was approximately 25' wide by 8' high. MSHA was notified within the prescribed time frame, but I failed to file a 7000-1 form. This is being sent to abate a citation.

An unintentional roof fall occurred in the 1st North Panel (#1 Unit 001) off the Main East in the #4 entry at the #24 cross cut +1440'. The fall was approximately 24' wide by 10' high. MSHA was notified within the prescribe time frame, but I failed to file a 7000-1 form. This is being submitted to abate a citation.

Struck by falling object

Was extending the survey sights in #1 room. A piece of rock fell out from in between the bolts hitting him in the head and back. Started missing work on 9/3/08.

Miner operator was loading coal, when a rock fell out between the roof bolts and struck him on the left shoulder and left leg.

Fall from machine

Truck operator missed a step trying to exit the truck and fell and injured hip.

Struck by flying object

Was struck by consolidated dirt while cleaning the bowl of his pan.

Over-exertion in pulling or pushing objects

He was hanging the bolter cable as the machine was moving around a corner, the cable hung on something and it jerked out of his hands.

Over-exertion in lifting objects

Lifting plant feed belt counter weight guard, had pain in left shoulder. Started missing work on 6/30/08

Fall down stairs

Nightime; shift was over; climbing stairs to contractor office carrying thermos; tripped; fell forward/hit the steps, rolling off underneath the stairs to the ground.

Caught in, under or between a moving and a stationary object

Employee was helping put up Heintzman beams. They had put a safety chain around the beam and the boom of the Bobcat, when one of the men pushed too hard, causing the beam to come off of the boom of the Bobcat. The employee tried to keep it from falling on another employee. When he tried to catch the beam, it smashed his lower right arm between the boom and the beam.

Individual was found lying face down by his haul truck unresponsive. CPR was performed and he was transported by ambulance to the hospital. He was pronounced dead at around 2:30PM 4/21/08. Cause of death was listed as Heart attack.

Struck by falling object

Removing a rock from the tail of the miner, when it flipped out of the conveyor striking him in the lower leg causing a small laceration. Started missing work on 12/10/08.
