Accident type, without injuries

The mine was shut down for the Holidays. There was no shift start time. No one was working. We had an 8" rain storm. Water entered the pit and eventually entered the mine openings.

An Unintentional roof fall occurred on the 3rd North Travelway. This happened between 18-19 crosscut.

Volunteer fireman notified Mine Management at 4:17 pm that there was a shed on fire at Red Hawk Mine. Shed was destroyed and storage trailer also received extensive damage. There was no one at the mine at this time. The fire was reported by a neighbor to 911. (There was no scheduled work and no one at the mine. System would not let me proceed without a shift start time.)

While examining, the employee was crossing over a belt crossover and strained his calf muscle while descending.

Operating Dozer and jarred lower back.

Employee was pulling on the battery nip for a coal hauler when he felt a pain in his lower back. He started missing work on 7/13/15.

Employee was recovering belt structure when he felt a sharp pain in his lower back.

Employee was pulling a piece of line curtain when ee tripped. EE twisted left knee. EE worked until he had surgery on 6/4/15.

Employee was loading battery cells onto a ride with a boom lift. He pulled on the strap to swing the cell onto the ride when he felt something pull in his left shoulder. The employee stayed at work until he had surgery on 6/29/2015.

Struck by falling object

Employee was roof bolting. He walked to the side of the bolter, when a rock 26"long x 8" wide x 5" thick fell from the roof striking him in the back of the head/ neck.

Employee was stepping off a bus to get on a man-trip and rolled his ankle.

Employee was putting a bolt in a hole he had drilled when he felt his shoulder pop and felt a sharp pain.

Employee ran over a large rock in the travel way in his ram car as he was going to dump on the belt. Employee hit his head and jammed his neck. Employee worked 3 days before going off work with injury.

Fall onto or against objects

Employee slipped and fell, hitting his face on a pipe. He continued to work until 1-20-2015. He returned to work on 1-23-2015.

Employee was climbing down ladder and his hand slipped, straining his arm. He started missing work on 1-17-2015.

Employee slipped and fell, hitting his head on truck bumper.

Employee was taking down Kennedy Panels and cut his right index finger on the panel. He received 4 stitches in his finger.

Employee was removing tap from hole. The wrench slipped and he cut his thumb on the bit. His thumb required 3 sutures.

Fall to the walkway or working surface

While moving a file cabinet in office, he fell causing a hairline fracture to his right ankle.

Employee chipped a tooth and got sutures in his lip when his mouth struck the steering wheel of a diesel ride.

Miner had hand on top of roof bolter and a rock fell out cutting his index finger requiring stitches.
