A roof fall measuring 19'Wx25'Lx6-8'Th occurred at this operation at xcut 57-58 in entry 7 of the intake/primary escapeway of the 2nd south. No one was injured and ventilation was not compromised.

A roof fall measuring 19'W x 30'L x 8-9'H occurred at this operation. Location is 3N/3ME #4 entry at 89+00 survey station. No one was injured and ventilation was not compromised.

A hot spot on a coal pile was discovered at the Prairie Eagle East Portal Raw coal pad. No one was injured. Steps are being taken to transport the hot material to the prep plant to be washed and taken from property.

A roof fall measuring 20'x20'x8' in height occurred at this operation. Location is 4E/4S/3ME 50+40 line room 5 1+20. No one was injured and ventilation was not compromised.

I.E. was loading belt rail frames on a sled. They tripped and fell. As they caught themselves, they felt a burning sensation and lump in their lower abdomen. I.E. continued to work full duty until surgery on 2-8-2024.

Over-exertion in lifting objects

I.E. was hanging continuous miner cable with other miners. As they were holding up the cable, I.E. felt a pop in their right wrist. Doctor's restrictions took the I.E. off work on 9-7-2023.

Over-exertion (Not Elsewhere Classified)

I.E. was setting up the CM to start cutting. As they were hanging cable across the entry they felt a sharp pain in their right shoulder. I.E. continued to work until a doctor took them off for surgery on 4-19-2024.

Struck by powered moving object

I.E. was knelt down rolling up a set of drop curtains they had just recovered. A battery scoop came through the LOXC curtain while leaving the face. The scoop did not see the miner and struck them in the back with the battery end of the scoop.

Over-exertion in lifting objects

I.E. was lifting continuous miner cable to help hang it from the mine roof. When lifting with their right arm, they felt a pop in their right shoulder. I.E. continued to work until a doctor took them off for surgery on 1-11-2024.

Fall from machine

I.E. was getting out of the passenger side of pickup truck at warehouse. As they were getting out of the truck, their foot caught a CB cord. They fell out of the cab on their right shoulder. I.E. continued to work until doctor took them off for surgery on the shoulder on 10-19-2023.

Fall to the walkway or working surface

OTR truck driver was walking around their truck checking lights. While walking the driver stepped in a hole causing them to trip and fall. When falling I.E. fractured two fingers on their right hand when hitting the ground.

I.E. was walking in a crosscut when they stepped on a rock with their left foot. I.E. felt and heard a pop in their left leg behind the thigh. Experienced pain throughout the leg. Doctor took the I.E. off work on 8-7-2023.

Caught in, under or between a moving and a stationary object

IE had just finished drilling a hole with the roof bolter. As they were taking their steels out of the roof, a section fell catching their right pinky finger between the bottom of the steel and the bolt wrench. IE had the finger looked at the next day and found a small fracture in the end of the finger. IE has not missed any work. *IE was taken off work for surgery on 5/31/23.

Struck by falling object

I.E. was attempting to put glue and bolt in a hole when the material they were bolting up broke and the two pieces struck the I.E. One rock measured 12"Thx39"x18". The other rock measured 8"Thx32"x16". Employee has continued to work but was placed on restricted duty on 3-16-2023.

Struck against a moving object

I.E. was driving ram car to the miner to get a load and ran over a rock with drive wheel. This caused them to injure their lower back and neck. I.E. has continued to work but received injections for pain on 03/07/2023.

Caught in, under or between running or meshing objects

I.E. was roofbotling with a 4' wrench. They added a 2' wrench for the height encountered. I.E. was pulling hand away when they hit the rotation lever. As they hit the rotation lever the end of their glove was between the two wrenches. The wrenches pulled their hand around the wrench causing a fracture to their left pinky finger. I.E. is working on restricted duty as of 11-21-2023.

Struck by falling object

I.E. was loading bolts on bolter tray when a piece of shale (18"x18"x2"thick) fell from the roof and struck the I.E. in the right forearm. This caused a laceration that required stitches. I.E. has not missed any work.

Caught in, under or between a moving and a stationary object

I.E. was driving roofbolter from cab while pulling up cable slack to last open. As they were backing around corner, they leaned back to see the light that was flagging them. As the I.E. leaned back they came in contact with the coal rib pinching the back side of their upper right arm. This caused two lacerations that required stitches. I.E. has not missed work due to the injury.

Struck by... (Not Elsewhere Classified)

I.E. was bolting a crosscut right. Foreign material got in behind the I.E.'s glasses going into their left eye. Material had to be removed by a doctor and eyedrops were prescribed. I.E. has not missed any days of work.
