An irregularity of transitional geology occurred during the bolting cycle.

Roof fall occurred at 3rd N, entry 6, between 33 and 34 cross cut. The area was approximately 3' thick, 18 feet long, by 15 feet wide.

Rock fell and struck employee on the left leg. Fracture to lower left leg.

Flying object (rock/coal/metal), struck the CM operator in the upper chest / lower neck region. The employee returned to work the next scheduled shift on restricted duty.

Twisted lower back while stepping out of a mud hole. Continued working regular duty till 7/21/16 when restricted duty began.

Employee stepped in hole and twisted left knee. EE continued working until ee had surgery on August 25, 2016. EE returned to work on August 29, 2016 on restricted duty.

Over-exertion in lifting objects

Picked up jack stand to move it and caused pain to lower back. EE continued working regular job until ee went on restricted duty April 29, 2016.

As bolter cable was reeled up, employee was struck by cable on right knee. EE had micro surgery on 9-1-16 and became reportable at that time. EE did not miss any work and is currently on restricted duty.

Employee was cutting bands on screen wire. Band came back and cut left hand. The reason for the delay in reporting was completion of the accident investigation. EE had micro surgery on February 4, 2016 and became reportable at that time. EE did not miss any work. EE is currently on restricted duty.

Struck by falling object

While installing a roof bolt, a sharp rock fell striking the roof bolter operator in the forehead above the right eye causing a laceration. The roof bolter operator received 7 stitches to the forehead.

Employee was loosening a pan lid and the lid shifted, cutting a finger. The finger required sutures.

Employee was cranking the suction line up and the crank slipped out of EE's hand and it struck EE on the back of EE's right hand between the thumb and index finger. EE received a fracture to EE's hand.

Driver cut arm on the ticket weather cover at the scale kiosk when the truck rolled as employee was getting their ticket.

Taking down kennedy stopping, cut pinky finger through glove, received 4 stitches.
