Caught in, under or between a moving and a stationary object

Employee was struck when a stair stringer, secure in 4 locations, inadvertently released, striking employee in the chest area, thereby forcing him into a Genie manlift located approximately 8-10 feet from the point of contact with the stair stringer.

Over-exertion (Not Elsewhere Classified)

Employee was stepping off of a ladder and twisted his ankle.

Fall from machine

Employee stepped off back of bolter.

Absorption of radiations, caustics, toxic and noxious substances

EE was plastering around a door and when he stacked another block on top of the door, the blockbond splattered up under his safety glasses and got under his safety glasses and into his left eye.

Struck against a moving object

Employee indicates that she was jostled while driving articulating truck causing injury to her neck and back. We were made aware of her injury when we received a fax from her attorney on 4/13/10. She did not inform her supervisor that she was injured.

Caught in, under or between a moving and a stationary object

EE was climbing on a scoop when it unexpectedly started moving. His leg/foot was caught in a chain that pulled it under the fender and wheel of the scoop.

On Jan. 28th,2010 at 1:45 a.m. EE got his right foot under the cat track of the beam setter as it was tramming forward. He was transported to the hospital for examination and x-rayed, nothing was broken, he was told it was only bruised and was sent home to keep it elevated. He started missing work on 01/29/2010.

Struck by rolling or sliding object

The mining machine was loading material into the shuttle car and a piece of rock slid across the load and was headed towards the employee in the cab of the shuttle car, he saw it coming and held his hand up to stop the rock, the rock cut his right hand, in the palm area at the base of his small (pinky finger). He went to the hospital and received 5 stitches and returned to work.

Struck by falling object

Employee was bolting at the face, putting up the center bolts, when the operator on the other boom lowered his boom, the wrench fell and struck the employee over his right eye, cutting his head above the right eye, he received 8 stitches, and returned to work.

Employee was cutting plastic off packed items, cut his left hand above the thumb. He went to hospital and received 9 stitches.

Struck against stationary object

Employee was tightening a bolt with a wrench, the wrench slipped off the bolt, causing the employee to punch the metal beam next to the bolt with his right fist, breaking a bone in his right hand.

Over-exertion in lifting objects

Employee was building an overcast and while lifting the piece of tin, he cut his right forearm. He was treated at the mine site and taken to the hospital and received 13 stitches. He returned to work, with no restrictions.

EE was making up Q-decking and handing it up to men working. A piece slipped and cut his right thumb.
