An unintentional roof fall occurred at cross cut 55, entries 1 and 2, in the tailgate #2. The fall in the number one entry was approximately 30 feet long by 20 feet tall, the fall in the number 2 entry took all of the intersection and crosscuts right and left for approximately 40 feet.

All incidents in 2017
This area was first found on Monday 5/8/2017 and at that time it was just a pot out. All 4 approaches were dangered off and floor to roof support was installed. The next day 5/9/2017 is when the pot out had fell out more and was then above the anchorage point.
We had an inundation of water on TG#2 at xc132. The water came from a borehole that was drilled for a turbine pump but was not installed yet. This was caused by the extensive amount of rain and flooding we received from a 100 year rain event from 4/28/2017-4/29/2017.
This was not a roof fall. A pot out occurred in South Mains primary escapeway between xc 76-77. This pot out was not above the anchorage point. This was reported due to the pot out being in the primary escapeway which impeded travel.
A roof fall occurred in HG5 xc 127 in entry 2. A roof fall clean up plan will be submitted and approved by MSHA before we clean up the fall.
Filling pod duster on the Long Wall monorail, was checking gasket when lid shut on employee's right small finger. I made a mistake, I didn't push the submit button for this accident, I have the copy in my folder.
EE pulled slick lines and hooked them up to a pump able crib, turned around and tripped over the slick lines and tried to catch self dislocating left shoulder.
I.E. was operating SandVick scoop hauling supplies to TG 2. Operator got left forearm in pinch point causing a fracture to both bones.
Employee was assisting in changing a 60"" top roller on HG#5's belt when EE smashed left middle finger. This resulted in a laceration and a hairline fracture on the tip of the left middle finger.
Employee was installing 60"" belt structure all day and wrist started hurting that evening at home. When ee woke up the next morning wrist was swollen and stiff. EE went to get it checked out and was informed ee had a fracture on left wrist.
Employee was assisting bolting partner by helping self put glue and a bolt in the rib, as ee turned to walk away ee heard a snap. When the employee turned around ee was struck in the left hand by a broken wrench. This resulted in a fracture to employees left hand.
This is being completed under protest to abate a citation. The IE alleged hurt back while pushing a roof bolt into the roof on 8/9/2017.
EE was re-claimimg vent tubing out of #3 entry. EE pulled on a tube that was laying on the floor & felt a pain in their back. EE continued to work until 08/21/2017. EE is off with back pain.
EE was building a stopping in a X-cut. When they lifted a concrete block up to stack it, they felt a pain in the left shoulder. EE missed the first day of work on 07/27/2017 due to the injury that happened on 05/09/2017. This report had already been submitted on 07/31/2017 but hasnt shown up in MSHA's system so I am resubmitting.
EE was in the process of knocking a hole in a stopping to push props through. As EE took a swing at the stopping with the sledge hammer the head of the hammer went through the hole causing EE to smash left ring finger between the block wall and the handle of the sledge hammer. This resulted in a fracture to the tip of left ring finger.
Employee was laying a block for the foundation of a stopping. As employee turned to pick up another block employee heard employee's left knee crack.
Employee was in the process of establishing ventilation to cut 2R. While ee was attempting to hang a round 4 way ventilation tube ee felt something pop in left elbow. This resulted in a tear to flexor tendon near the elbow in left arm. Surgery to repair the tendon was performed on 4/6/17.
I.E. was pumping water in the bleeder. Felt pain in right ankle while walking and EE's foot started hurting. EE didn't report the incident to anyone until 03/08/2017. EE went off for unrelated to work back surgery on 07/17/2017. We were informed on 07/28/2017 that the Dr. connected the ankle injury to the back surgery that EE had went off for.
Employee was dragging waterline down the HG5 beltline when ee stepped on a chunk of coal causing self to twist right ankle. This resulted in a sprain.
Employee was in the process of removing the bolts from the right side tram motor on a Double Boom Roof Bolter, when employee's ratchet slipped causing employee to jam employee's right arm on the mine floor. This resulted in a strain to employee's right elbow. Employee continued to work up until 3/6/2017.
Employee was roof bolting and had pulled roof mesh onto the ATRS. Employee was holding on to it to stabilize the mesh when a rock 2' X 2"" X 2"" thick hit employee on the back of the hand.
While in the process of restarting the Ref #4 belt a 3 ton com-a-long on the take up broke striking Employee on the left side of employee's face. Employee was standing approximately 25 feet back from the take up.
This is being filled due to a citation issued during a Part 50 Investigation. On 7-17-2017 the IE was removing trash from a xc and tripped over a concrete block. While trying to catch themselves from falling. The IE extended arm out and came in contact with the coal rib resulting in pain to shoulder. The IE worked until 2-28-2018 and then had shoulder surgery.
Employee was prepping surface belt for vulcanization. The employee removed bottom belt roller and smashed right thumb. Restricted Duty.
This is being filed under protest to abate a citation. The IE was pulling drill steels out of the roof and felt a pain in EE's right arm
Removing bottom roller pinching left index finger between bottom roller bracket and rail.
The IE stated to the operator IE was struck by a rock that fell from between the shields and struck hand. Stated rock was 6""x 8""x 2"" and IE's hand was on the cable tray.
IE was walking in muddy area and tripped over hog panels that were lying in the mud. IE fell on side straining back and hurting ribs.
Employee was cutting a cap board with a knife to fill a small hole in the stageloader to prevent coal from spilling out. While cutting the cap board the knife slipped and employee cut their upper right leg. This resulted in employee receiving 3 stitches.
IE was handling hog panel and while passing a piece of it to bolting partner, IE felt a pop in shoulder
Employee was trying to remove the pot from a miner. While employee was hitting the foot shaft with a hammer and chisel a piece of metal struck ee in the right forearm. Employee had the piece of metal removed and received 3 stitches.
Employee was in the process of throwing over cable when a section came back and hit employee on the right side of employee's lower right leg. This caused employee's right leg to twist and employee heard employee's right knee pop. This resulted in an ACL tear.
IE twisted knee when IE fell while making a weekly examination.
EE started down the slope and ee stated the ride started sliding. EE tried to control the ride with the brakes. When ee still didn't feel like ee had control engaged the E-Brake. EE then steered the ride to the solid side rib and struck an I-beam, this caused the ride to stop. EE had a laceration above left eye and the side of left wrist that received stitches.
Employee was in the process of breaking up a rock with a ball peen hammer and a proto chisel at shield #9. A sliver of metal broke off the head of the hammer and struck ee in left forearm. This incident became reportable on 8/9/2017 when the employee had to have the piece of metal removed from arm.
Employee was tramming a scoop out by off the section that still had a flinger duster attached to the bucket with 2"" ratchet straps. While tramming out by one of the straps broke and the metal hook from the strap came back and hit employee in the upper lip. This resulted in the employee receiving stitches.
I.E. was unlatching a 5th wheel trailer from the tractor ee was driving. The pin that locks the tongue in place was stuck so ee tried to loosen it by hitting it with a hammer. After it had freed up ee pulled the pin out, the spring pulled it back in which pinched thumb between the plate & tractor. I.E. had to have procedure done to fix thumbnail. No work was missed.
Employee was standing on the 6th step of a 12' ladder to drop down a belt chain that was tied up to the rib. Employee was leaning against a 4"" pipe that was about chest level while on the ladder. The pipe moved some causing employee to lose employee's balance and fall. This resulted in a broken right wrist.
Employee was helping roof bolter's with supplies. EE cut the metal band that held the roof mesh together & the band sprung out & struck ee in the right cheek. EE received 10 stitches to right cheek the following day. EE has not missed any work due to this injury.
I.E. was getting wire mesh loaded on tray of roof bolter. EE had hand resting on the mesh tray and that's when partner cut the band on the wire without ee knowing. It landed on finger which caused a laceration on ring finger. It required 2 stitches & ee missed no work.
Employee had just finished putting up a roof bolt and turned to re-load tray with material. When ee turned,ee then struck the back of arm against a stack of roof bolt plates. It caused a laceration about 1"" long that required 4 stitches. Employee returned to work after stitches were put in.
Employee was heading down the Longwall face from the tailgate towards the Headgate. EE raised up while ee was walking and came into contact with the side of a shield. This resulted in employee receiving 7 stitches.
Laceration of inner left thigh due to razor knife. Injury required sutures to close wound.
Employee was cleaning out tip of sprayer, pushed down too hard on the tip and it sprayed employee in the face and left eye. This resulted in employee receiving prescription eye drops.
Employee was crossing over the Ref belt on a catwalk to get to the classifying sump on B side. While employee was traveling over employee's hard hat came into contact with the discharge pipe of the spiral ref screen causing employee to chip a tooth.
I.E. was hanging a vent tube up after it had been knocked down. A piece of rock measuring 4"" X 5"" X 1"" thick hit EE in the mouth causing EE to have 4 stitches in the upper lip.
Employee was changing bit sleeves on a continuous miner drum. A piece of metal from the hammer hit EE in the left forearm. EE thought it was just a small cut but later found out the small chip was in there. It was not reported until 2/28/17 when notified from Dr.
Employee was hanging curtain in the face. EE slipped & fell from the 3rd step of an 8' ladder. It caused a very slight fracture to a bone in left forearm.
Employee was driving a diesel scoop & ran over a concrete block. When it jarred ee, it caused a crack in a back tooth on right side of mouth.
EE thinks the pain in their hands may be carpal tunnel syndrome, EE believes this was caused from repetitive actions.
