Accident type, without injuries

We had a roof fall at x-cut 101 in the belt entry #1. It was 27 feet long, and about 10 foot high. This is on the longwall unit, Headgate #2, 004 MMU.

It was about 3:00 PM when we found a roof fall at x-cut 66 in the number 3 entry of the Headgate #3 unit.

The examiner found a roof fall at x-cut 93 in the main north in entry #7 while doing his weekly exam.

An unintentional roof fall occurred at 10:30 PM inby x-cut 21 in the Headgate #2 in the number 3 entry. It is 14 feet wide by 14 feet on the left side, it is about 9 feet high.

We had a roof fall in the Headgate #2 bleeder entry #2 between x-cut #9 and #10.

We had a roof fall in the #2 entry between the # 3 and #4 x-cuts of the Headgate #2 set-up rooms. MMU003 unit.

The jumper lead on the hoist motor rotor at the connection point shorted out, causing a hole to be burned in the end of the rotor. The motor was removed and taken to a motor shop, disassembled and repaired. The motor was reinstalled and running on 01/27/2013.

Struck by falling object

Employee was shoveling coal into the Longwall pan line. A piece of coal fell from the face striking the employee. Employee was standing under shield #123 on the Longwall face at the time of the accident.

Over-exertion in lifting objects

Employee stated he lifted drill feed jack, and he twisted his body and trunk and felt a tingling sensation in his back. He continued to work until 12/17/2013 when he started to miss work due to back injury.

Employee was traveling down the slope to his job site and a piece of tin shedding fell and hit him in the head.

Employee was checking his drill pod for the suction and the drill steel fell out of the roof onto his left pinky finger. He has a fracture of his left pinky finger. He was working on the Headgate #1 Unit.

employee was installing the rib bolt and the rib rolled out, striking the employee on the left foot fracturing his left foot.

Employee was taking a boomer off the bantom duster out of the scoop bucket, when the boomer came loose it hit him in the back of the left knee, he thought he would be OK, the x-ray yesterday showed a torn meniscus, he will be operated on his left knee.

The employee was walking along the longwall face and the face of coal rolled out on the panline and struck his left foot, he was wearing rubber boots and coal punctured his boot. He went to the hospital and received stitches in his left foot.

The injured ee was kneeling down and placed his hand on a sheeting gurt so that he could peak over the edge to talk to an ee and when his body weight shifted on the sheeting gurt it sprung down and pinned his left hand between the sheeting gurt the floor and toe plate.

Fall from machine

Employee was standing on the platform while the ATRS was lowered to put up the last row of bolts, when the ATRS lowered a rock fell ahead of the boom and knocked the operator off the platform. He fell 3 feet to the ground and landed on the piece of rock. He had lacerations to his face and left arm that required stitches. His face had broken bones.

Employee was hauling a load of concrete blocks down the slope A with a fletcher tractor and trailer. As he turned at the bottom of slope A- he slid and hit the corner at the bottom of the slope A. He was taken to the hospital, he broke his femur bone on his left leg, they operated on his leg and put a pin in it. He is being released from the hospital today 5-10-13.

Absorption of radiations, caustics, toxic and noxious substances

Pouring concrete. Line on chute came off hitting employee in face. Concrete got in his right eye and ear canal.

Employee was reaching for a roof bolt plate on the back of the roof bolter and the stack of plates shifted and fell on his left thumb. Resulting in a broken left thumb.

Employee slipped and fell while holding drill, sprained left knee.

Drilling in shaft bottom, pulled steel, strained right shoulder.

Employee was installing angle brackets with a roof bolting machine, while putting the roof bolt into the hole, he heard a pop in his right elbow. He was working in Headgate #2 unit in the number 2 entry. He has continued to work until July 11th, after seeing his doctor, he was put on restrictive duty, and has been off since that time.

Struck by rolling or sliding object

Employee was helping the miner operator move into a cut to mine, as he threw the cable over against the right rib out of the way, the rib rolled out and hit him on his right side. He twisted his back, bruised his right shoulder and cracked his ribs. He was taken out of the mines, and went to a hospital for x-rays.

Employee was making the faces on the section in x-cut left in the number 2 entry at x-cut 26. He walked up on the left side of the roof bolter and checked the top where the ATRS was located, and he was struck by a piece of rock, it bent his back down, and he injured his lower back.

Employee was stepping out of the Bobcat and he thought he twisted his ankle, when checked at the hospital he had a broken left foot. He was located at x-cut 24 of the Headgate #3.

Employee stepped through a mandoor of a stopping and he felt his left knee pop. He continued to work everyday until 5/16/2013 when he had surgery on his left knee to repair ACL damage, the surgeon put staples in his knee. He came back to work on 5/17/2013, but knee surgery was performed on his left knee.

Employee was walking at the bottom of the shaft and slipped, as he slipped he grabbed the screen wire to catch his balance, he strained his left hand ring finger in the mesh, he strained his left ring finger, he continued to work and on December 20th had it operated on, then came back to work.

Employee was putting up the angle brackets with the single boom roof bolter he dropped the platform and a steel plate shot out and hit him in the left forearm, he went and received 6 stitches and returned to work.

Employee was putting accutrak in panline and his left forefinger got caught under the accutrak. Employee fractured his left forefinger.

Employee from Foresight Supply was working in the warehouse area and he smashed his right ring finger between two pieces of metal, he went to the Benton hospital and received two stitches.

Flash burns (electric)

Employee was hanging the shuttle car cable for a shuttle car tie-off, when he bent the cable double the cable arced and flashed, the cable had a pin hole in it, the cable flash burned his left inside forearm area. He was taken to the hospital to have the burn area cleaned and wrapped. He received a 2nd degree burn to the inside of the left forearm area.

Employee was walking down the face of the longwall, he tripped and fell and his left hand hit the panline and a rock fell against the panline and hit his hand. He went to the hospital and received 6 stitches in the palm of his left hand and returned to work.

Employee was busting rock with a sledge hammer, and when he hit the rock a piece of the rock flew up and hit him under the lip. He received 2 stitches under his lip.

Headgate number 1 unit, employee was installing a roof bolt and the steel plate slid down the bolt and cut employee on top of the right wrist. He went to the hospital and received 3 stitches and returned to work.

Struck against stationary object

cover on Tensioned box on Head #1 gear box got knocked down on employees hand as he was re-routing hoses. Hitting right hand.

Employee was cutting a plastic zip tie off his knee pad on the surface to take a shower and go home, as he cut up on the zip tie he finished cutting the zip tie and ran a box cutter knife across the top of his left hand. He went to the emergency room and received 5 stitches in the top of his left hand and came back to work the next day.
