We had a roof fall at x-cut 84 in the number 1 entry of the Headgate #1 Unit. This is the Longwall unit 004 MMU.

An unintentional roof fall occurred at this operation at 13:45 at X-cut #7 between #4 and #5 entries at the mouth of the Headgate #2 unit.

We had a roof fall in #1 entry at x-cut 111 of the Headgate unit 004 mmu. We submitted a fall clean up plan, we did the plan and the area was released by MSHA on 07/06/2012 to resume normal mining operations.

Roof fall at x-cut 111 on the Headgate #1 unit between the number 2 and #3 entry in the x-cut. It did not block the ventilation or the travel past it. The fall measured about 18 feet wide by 20 feet long, and fell out about 8 to 9 feet high. We set more standing support under and around this area. In the form of cans.

We had a small potout on the Tailgate at x-cut 86 and a half in the number 3 entry. It was 6 feet wide by 6 feet wide by 5 foot high, and it did not fall ABOVE or at the anchorage point of 8 feet, and it did not pull any bolts. I am filling this report due to it being called in to MSHA on the 1-800 746-1553 number before i got to go and inspect the fall myself.

An unintentional roof fall occurred at this operation at x-cut 54 in the number 2 entry of the tailgate unit 002 in the intersection.

An unintentional roof fall occurred at x-cut 93 between 1 and 2 entry of the Tailgate Unit 002. It is not blocking the escape way or the ventilation off the unit.

Work deck was wet due to rain. Employee slipped while installing bolt/drill got away from him and pinned employee against hand rail.

Employee was greasing belt rollers and bearings when the grease hose got wrapped around his right arm. He broke his right forearm. He was located at the Headgate # 2 tripper drive.

Fall to the walkway or working surface

Employee was prying the cap out of a planetary on a roof bolter, he slipped and fell and twisted his left knee. He continued to work until he could not stand the pain any longer, he scheduled meniscus surgery on 11/20/2012, he has been off work since the surgery.

Employee was moving drill and steel from one hole to another. Strained back.

Struck by falling object

Clean shaft bottom. Rock fell from rib and hit employee on left shoulder causing head contusion with secondary cervical strain

Employee was running a bobcat and there was some cables hanging down in front of him and he tried to raise them up with his left hand and mashed his left hand between the bobcat and roof above him. We took him to the hospital and he had 2 pins put in his left hand.

Employee was backing up with the D-10 dozer and backed into feeder #3. He was taken to the hospital and had broken ribs and a bone broke in his collar.

Employee was running the bobcat and when he was getting back in the seat he hit his head on the front of the equipment jamming his neck.

Over-exertion in pulling or pushing objects

Unloading excavator from tractor trailer. Pulling on sling. Strained upper and lower back

Fall to the walkway or working surface

Employee was inspecting the screen deck on a scalping screen for any missing screen panels, and fell backwards hitting the back of his head, he cut the back of his right side of his scalp, he was taken to the hospital and received stitches to the back of his head.

Over-exertion in lifting objects

Employee was hanging high voltage cable when he felt a sharp pain in his right shoulder. He finished working and he said it kept hurting him while he continued to work, he did have surgery on his right shoulder on 11/20/2012.

Employee states that he had back surgery before coming to this mine, he said while lifting belt rollers he felt like his back was beginning to hurt him. He said he was going to go to his doctor and have it checked out to see if his back fusion was acting up on him. His doctor was scheduling an appointment. Status unknown at this time.

Struck against stationary object

Employee was working on the longwall section and he came in contact with a piece of screen wire that was sticking down between the shields and cut his left cheek. He went to the hospital and received stitches in his left cheek.

Employee was roof bolting and installing the angle brackets and a piece of rock fell out of the roof and struck him on the right forearm. He went to the hospital and received 4 stitches in his right forearm.

Employee was running a drill and a rock fell out and hit him on top of the right hand and cut his hand between the ring finger and the knuckle. He went to the hospital and received 2 stitches and returned to work.

The mechanic was using a come-along to re-connect the shuttle car conveyor chain back together and the hook slipped and hit the mechanic in the mouth area under his nose. He was taken to the hospital and received stitches in his lip.

While removing a hose from a P-70 water pump, the end of the hose caused a laceration over the right eye brow, we took him to the clinic and he received 6 stitches and returned to work.

Employee was reaching to grab the drill steel from the drill pot, and he ran his right foreman on the corner of the steel plate lacerating the bottom of his right forearm. He went to the hospital and received stitches and returned to work.

Struck by falling object

Employee was putting up roofbolts and his drill steel got caught in the hole, when he tried to pull it out of the roof, when he put his hand under the drill steel it fell out of the hole and smashed his right ring finger on his right hand. He went to the hospital and received 3 stitches and returned to work.

While changing cat tracks on the Bobcat, he was removing, something hit his left middle finger, and fractured the end of his middle finger in the last section of his finger. He thinks it was the grease fitting.

Employee was installing the wire mesh to the roof and rocks fell out smashing his fingers in the screen wire and the ATRS on the roof bolter. He went to the hospital and received 5 stitches in his right hand on top of the knuckle of his pinky finger.

Employee was hanging curtain on the unit and a sharp rock fell and hit him in the left cheek of his face. This happened in the #2 entry of the Headgate #2 Unit. He went to the hospital and received 9 stitches on his face and returned to work.

Struck by flying object

Employee was using a sledge hammer and busting up a chunk of limestone rock, when he hit the rock a piece a rock hit him in the left leg, in the shin area. He went to the doctor and he put 3 stitches in his left leg and returned to work.

Struck by falling object

Employee was putting up screen wire and a rock slid out of the screen wire and hit him in the upper left lip area. He was taken to the hospital and received 4 stitches.

A piece of rock fell off of the ATRS while installing screen wire and hit the employee on the back of the head and on the left hand. He went to the hospital and received stitches in his left hand middle finger, and returned to work.

Employee caught his left shoulder on a piece of metal. He received three stitches in his left shoulder, and returned to work.

Employee was using a razor knife while cutting, he ran the blade of the knife across his left hand behind his left thumb. He went to the hospital and received 6 stitches in his left hand behind his thumb, and returned to work.

Fall onto or against objects

Employee was loading belt structure and slipped on the trailer, cutting his right shin, causing a deep laceration on his right leg. He went to the hospital and received 11 stitches and returned to work.

Caught in, under or between a moving and a stationary object

Employee was putting a drag on a shuttle car, when he lifted one end and another employee lifted the other end, the drag smashed the end of his middle finger on his left hand. He was taken to the hospital and received 12 stitches in his finger and returned to work.

Struck by falling object

Employee was installing equipment doors on the belt line when a concrete block fell above him and struck him on the left forearm causing a laceration, he was taken to the hospital and received 8 stitches.
