Struck by falling object

Employee was walking at the plant facilities, a cornerstone of the crane collapsed, hitting the employee directly to the body and falling down. Employee was wearing safety equipment as required (capacete). Received multiple injuries to the upper and lower body.

Struck by... (Not Elsewhere Classified)

EE was said that he was holding a nail with a wrench then he started hammering the wrench it slipped and hit between his left thumb and index finger. It caused a laceration and his hand to swell up.

Struck against a moving object

The employee alleges that while unloading material in the hauling truck in the crusher primary area he injured his left leg, left shoulder and left arm and elbow.

Over-exertion (Not Elsewhere Classified)

The EE was getting of the excavator from the chain to the floor and felt pain in the lower back.

Caught in, under or between a moving and a stationary object

Employee alleges that while working with a wrench, it fell and pinched his right hand causing an injury between his pinkie finger and his right finger. Five(5) stitches were taken

Struck by flying object

The employee was in the secondary plant repairing a pump. He hurt his hand with a piece of metal that flew when he was hammering. The metal penetrated the skin on his left hand. He felt a strong pain in his left hand.
