On September 22, 2025, an individual entered the facility's parking lot and illegally stole an employee's vehicle. The individual took the wrong exit and headed toward the plant area. The employee confronted them but was struck in the back of the head with a drill found in the vehicle, causing a minor injury. The individual was later arrested in the nearby community.

The employee alleges that when employee got off employee's heavy equipment, employee stepped on a stone from the internal road, causing pain in right knee.

Employee was cleaning a continuous corridor to a vibrator screen, when an approximately 2-inch stone struck employee's right shoulder, causing a bruise.

While the employee was cutting a strap of employee's truck with a knife to replace an hydraulic hose, the thumb of employee's left hand was cut.

Employee alleges that EE was tightening the oil tube bolts on the 320B excavator bucket. When EE was adjusting one of the Allen screws, the tool came loose causing the ring finger of EE's left hand to hit the metal, causing swelling in the finger.

The employee alleges that while filling the truck's tank with diesel the hose of the air pump that draws diesel from the tank came loose, spreading diesel down the ladder, railings, and floor of the truck. When the employee tried to get down, EE slipped hitting EE's groin, thigh and knee on the right side of the body.

The employee received a small cut in left hand while making a movement of a tire of the backhoe loader. Employee received 5 stitches at a diagnostic and treatment center. Employee will return to work on August 9, 2019, as notified.

The employee indicates a painful back and apparent pinched nerve, allegedly from operating the excavator 320. The employee was referred to Fondo del Seguro de Estado by a private doctor.

The employee was accommodating the Plant Conveyor Toughing of conveyor #6 (with an approximate weight of 10 to 15 lbs.). When he forcefully pushed to accommodate it into place, he felt discomfort in his neck and lower back.

The employee was giving maintenance to the conveyor; he was standing in the hallway holding the interior part of the handrail of the conveyor. He got dizzy, falling on hallway, hitting his right leg and affecting the muscle of the thigh.

The employee was repairing the primary feeder chain. The chain was twisted. When he tried to straighten the chain, it was loose on the bottom. When it stretched, it moved trapping the little finger of the right hand against the sproket.
