EE alledges that while removing an engine part from a loader he made an effort to loosen the screws when he felt a pain in his lower back.

The EE alleges that while he took the stand that holds the strut hydraulic hammer with a 16-pound brown, he felt a sharp pain and cramp in his right arm.

The employee alleges that while unloading material in the hauling truck in the crusher primary area he injured his left leg, left shoulder and left arm and elbow.

The EE was getting of the excavator from the chain to the floor and felt pain in the lower back.

Employee alleges that while working with a wrench, it fell and pinched his right hand causing an injury between his pinkie finger and his right finger. Five(5) stitches were taken

The employee was in the secondary plant repairing a pump. He hurt his hand with a piece of metal that flew when he was hammering. The metal penetrated the skin on his left hand. He felt a strong pain in his left hand.

The EE was doing some maintenance work on the conveyor belt #8. He was using a knife to cut the belt, when it slipped and gave him a deep wound on his right leg.

The EE was parking the truck on the premises of the company. While he was getting off the truck, he hurt his right ankle on a mountain of dirt. He felt a strong pain.

The EE was unscrewing bolts in order to change the converyor belt. When he loosened the bolt, the wrench hit him in the nose and right eyebrow, cut his skin.

The employee was placing a beltscale in the conveyor with another employee. While lifting a metal roller it turned over pinching his right ring finger causing an open wound.

The employee was getting off the loader by the stairs when he slipped and fell injuring his back.

The EE was bringing the mine loader to the office area. As he bent down, he slipped on the stairs and fell on them. The EE feels lots of pain in his lower back side and his right leg.

The employee was cleaning the conveyor with a pressure hose when it slipped. The pressure of the water impacted his right eye, removing the retina.

EE alleges that during the week of 24 to 28 of July he was carrying a screen cloth that he placed on the Secondary Vibrator which has different weights. He injured his back and his right thigh.

the employee was cleaning a welding & some particles got into his right eye.

The employee was lifting up some acetylene tanks to put them in the loader and injured his back.

The employee was climbing the stairs and did not step firmly, which caused an injury in his right knee with the sharp stairs. They took four stitches on his knee.

The employee was going up the stairs to verify the surge tank level. Before reaching the top he turned around and his hands slipped and he fell setting down between the tank pump. He injured his back.

THE EMPLOYEE WAS PULLING THE STRAP TO PLACE IT IN THE CONVEYOR. HE FELT SUDDEN PAIN IN HIS LOWER BACK UP TO THE WAIST.

The employee was connecting a special pole in the sub-station of the secondary. The pole rebounded and struck the right part of his face. He had a small cut in his mouth.

While the employee was repairing a piece of machinery with a hammer, he hit a piece of metal, which caused an injury to his fourth finger from the right of his left hand.

THE EMPLOYEE WAS ADJUSTING THE SCREWS OF THE PRIMARY CRUSHERS BLADES, A PART OF THE WRENCH DISASSEMBLED AND IMPACTED THE MIDDLE FINGER OF HIS LEFT HAND

THE EMPLOYEE WAS CLOSING A COMPARTMENT IN THE CRUSHER. UPON PRESSING FORWARD WITH STRENGTH, HE FELT AN EXCRUCIATING PAIN IN HIS RIGHT SHOULDER.

The employee was cleaning a machine, which he had mistakenly forgotten to turn off. The machine activated itself, cutting the index finger of his right hand.

THE EMPLOYEE WAS CLEANING THE FLOOR UNDER THE PRIMARY PLANT SCREEN, REMOVING EXCESS MATERIAL WITH A MOBILE EQUIPMENT ""BOBCAT"". A PVC WATER PIPE WAS BROKEN AND THE EMPLOYEE UNBUCKLED HIS SAFETY BELT AND INCLINED HIS BODY UP TO LOOK AND AT THAT MOMENT THE UPPER BUCKET FELL TRAPPING HIM BETWEEN THE BUCKET AND THE FRONT HOOD EQUIPMENT. HE RECEIVED CONTUSIONS TO CHEST, SHOULDERS & BACK.

THE EE WAS GOING DOWN THE STAIRWAY TO EMPTY THE GARBAGE, UPON STEPING ON THE GROUND SHE SLIPPED AND FELL FORWARD, HURTING HER CHEST, RIGHT ARM, RIGHT LEG, THE KNEES AND BOTH HANDS.

THE EE WAS CUTTING METAL & A PIECE FELL ON HIS PANTS PERFORATED THEM CAUSING THE METAL TO FALL INTO HIS LEFT BOOT & BURNED AN AREA OF HIS FOOT.

THE EMPLOYEE ALLEGES THAT WHILE HE WAS CLIMBING ONTO THE BULLDOZER, AS HE GRABBED ONTO IT HE FELT A PROFOUND PAIN IN HIS RIGHT HAND WHICH MADE HIM LOSE HIS BALANCE. HIS RIGHT HAND IS VERY SWOL LEN AND HE FEELS A LOT OF PAIN. HE CANNOT CARRY ANYTHING THAT IS TOO HEAVY WITH THAT HAND.

THE EMPLOYEE ALLEGES THAT WHILE HE WAS INSTALLING A RADIATOR THE JACK HE WAS USING SLIPPED AND FELL ON HIS LEFT MIDDLE FINGER.

THE EE WAS INSTALLING A MOTOR MOUNT IN A TRACK DRILL WITH A JACK. THE JACK SLIPPED AND THE MOTOR FELL, CUTTING HIS LT. HAND MIDDLE FINGER AND HIS RT. HAND THUMB. (WE DON'T HAVE FACILITIES FOR THIS KIND OF JOB).

THE EMPLOYEE WAS DRIVING A TRACK DRILL UP A HILL, THE EQUIPMENT ALMOST TIPPED OVER, HE JUMPED OFF FALLING ON A LOOSE ROCK HURTING HIS LEFT KNEE.HE FEELS A LOT OF PAIN ON HIS LEFT KNEE.

THE EE ALLEDGES THAT WHILE HE WAS HOSEING DOWN A CONVEYOR BELT, A STONE FELL ON HIS RIGHT HAND HURTING AND BROSING THE HAND.

THE EE WAS PAINTING AN OFFICE, HE WAS STANDING ON A LADDER WHEN HE LOST HIS BALANCE AND FELL. HE INJURED HIS WAIST. HE CLAIMS TO BE IN ALOT OFPAIN ON THE BACK SIDE OF HIS WAIST.
