The EE was doing some maintenance work on the conveyor belt #8. He was using a knife to cut the belt, when it slipped and gave him a deep wound on his right leg.
The EE was parking the truck on the premises of the company. While he was getting off the truck, he hurt his right ankle on a mountain of dirt. He felt a strong pain.
The EE was unscrewing bolts in order to change the converyor belt. When he loosened the bolt, the wrench hit him in the nose and right eyebrow, cut his skin.

THE EMPLOYEE WAS CLOSING A COMPARTMENT IN THE CRUSHER. UPON PRESSING FORWARD WITH STRENGTH, HE FELT AN EXCRUCIATING PAIN IN HIS RIGHT SHOULDER.

A PVC WATER PIPE WAS BROKEN AND THE EMPLOYEE UNBUCKLED HIS SAFETY BELT AND INCLINED HIS BODY UP TO LOOK AND AT THAT MOMENT THE UPPER BUCKET FELL TRAPPING HIM BETWEEN THE BUCKET AND THE FRONT HOOD EQUIPMENT. HE RECEIVED CONTUSIONS TO CHEST, SHOULDERS & BACK.

THE EE ALLEDGES THAT WHILE HE WAS HOSEING DOWN A CONVEYOR BELT, A STONE FELL ON HIS RIGHT HAND HURTING AND BROSING THE HAND.
THE EE WAS PAINTING AN OFFICE, HE WAS STANDING ON A LADDER WHEN HE LOST HIS BALANCE AND FELL. HE INJURED HIS WAIST. HE CLAIMS TO BE IN ALOT OFPAIN ON THE BACK SIDE OF HIS WAIST.
