ROOF FALL OCCURRED AT 48 + 37 BETWEEN 1 SOUTH & 2 NORTH ON OLD TRACK ENTRY. #5 ENTRY MAIN EAST. FALL WAS 14'HIGH, 20'WIDE, AND 60'LONG. IT WAS INITIALLY MINED JANUARY 1970.
ROOF FALL OCCURRED AT 11 + 00 ON MAIN EAST TRACK BETWEEN FISH CREEK PORTAL AND 1 SOUTH. CROSSCUT HAD PREVIOUSLY FALLEN AT THIS LOCATION. FALL WAS 15'HIGH, 20'WIDE, AND 35'LONG. IT WAS INITIALL Y MINED IN MARCH 1969.
ROOF FALL OCCURRED AT 9+55 ON TRACK ENTRY (#4 ENTRY) BETWEEN FISH CREEK PORTAL AND 1 SOUTH. FALL WAS 9'HIGH, 20'WIDE AND 40'LONG. IT WAS INTITIALLY MINED IN MARCH 1969. OLD MAIN EAST TRACK ENT RY.
4 NORTH RETURN AT 63+13(NEAR 5 RIGHT-4 NORTH LONGWALL TAILGATE RECOVERY AREA) ROOF FALL. INITIALLY MINED AUGUST 1999. FAILURE AT INTERSECTION AND PART OF ENTRY. AREA 100 FT LONG, 16 TO 17 FEET WIDE, APPROX 15 TO 20 FEET HIGH. DAMAGE TO EQUIPMENT ONLY.
2 RIGHT 5 SOUTH TRACK ENTRY, TWO EES WERE INSTALLING FISH PLATES IUNBY A FLAT CAR LOADED WITH RAILS AND ATTACHED TO A BATTERY LOCOMOTIVE. WHEN A THIRD EE ATTEMPTED TO NMOVE THE LOCOMOTIVE AND RAIL CAR OUTBY, IT TRAVELED INBY STRIKING THE EES INSTALLING THE FISH PLATES RESULTING IN FATAL INJURIES TO ONE EE AND MINOR CONTUSIONS TO THE OTHER EE. ACCIDENT IS STILL UNDER INVESTIGATION.
2 RIGHT - 5 SOUTH LONGWALL SECTION; MAN WAS OPERATING A SCOOP CAR. AS HE WAS GOING AROUND A TURNHIS DOOR CAME OPEN. HE STARTED TO GRAB THE DOOR AND HIS LEFT HAND WAS CAUGHT BETWEEN A PIVOT JAC K AND FRAME OF THE DOOR.
2 RIGHT-5SOUTH- BELT ENTRY. BELTMENT WERE GETTING READY TO PUT BELT ON BELT STRUCTURE. MAN WAS USING COMALONGS TO RAISE A ROLL OF CONVEYOR BELT SO IT COULD BE UNREELED FROM THE ROLL. AS MAN WA S EXERTING PRESSURE ON THE HANDLE, THE MECHANISM SLIPPED AND HE HIT HIS RIGHT THUMB BETWEEN HANDLE AND THE ROLL OF BELT.
1 RIGHT 5 SOUTH LONGWALL BELT LINE. MAN WAS TAKING BAT WING OFF OF MONORAIL BOGGIE WHEN THE BOLT CAME OUT OF THE CHAIN BETWEEN BOGGIES THIS LET THE BOGGIES THAT HOLD LONGWALL CABLES ALONG MONO RAIL GO BACKWARD VICTIM GOT HIS LEFT HAND AND FINGERS CAUGHT BETWEEN WRENCH THAT HE WAS USING A ND A MONORAIL HANGER.
AS CO-WORKER CUT DOWN TREE THE WIND BLEW INTO INJURED, STRIKING HIM ON THE CHIN. INJURED THEN LOST HIS BALANCE AND FELL DOWN A HILL INJURING HIS ANKLE.
ALONG #10 MAIN LINE BELT- BETWEEN 29 AND 30 CROSSCUT. HIGH AREA PREVIOUSLY RIB OF STONE AND COALHAD FALLEN OUT. MEN WERE MESHING AND BOLTING BAD RIB. THEY WERE 85% FINISHED. EMPLOYEE WAS USING A SLATE BAR TO SCALE A SMALL AREA WHEN A LARGER AREA FELL OUT HITTING HIM.
ALONG #10 MAIN LINE BOLT-BETWEEN 29 & 30 X-CUT HIGH AREA, A LARGE ROB HAD FALLEN OUT. MEN HAD BEEN BOLTING AND MESHING IT. 85% OF THE AREA HAD BEEN DONE. EMPLOYEE WAS USING A SLATE BAR TO SCA LP A SMALL AREA WHEN A LARGE AREA FELL OUT HITTING HIM.
OLD DUMP EMPTY TRACK ON TURN, MAN OBSERVED A LOW RAIL ON TURN. HE STOPPED HIS ROCKDUST TRIP AND WAS GOING TO RAISE TRACK AND BLOCK IT. WHEN HE GOT OUT HIS LIFTING JACK, IT WAS PLUGGED WITH MUD. HE TURNED THE JACK OVER AND HIT IT ON STEP OF LOCOMOTIVE TO REMOVE MUD. THE STEM CAME DOWN CATCHING THE MAN'S LEFT RING FINGER BETWEEN THE BASE AND THE FOOT OF THE JACK.
CREW HAD MOVED SECTION TRANSFORMER AND PARTS CARINTO A CROSSCUT. VICTIM WAS STANDING NEAR CORNEROF CROSSCUT. A SHUTTLE CAR WAS ANCHORED ON PARTSCAR. SHUTTLE CAR PARKED OUTBY CORNER BEGAN TO RE EL HIS CABLE UP. CABLE HIT VICTIM ON LOWER LEG.
1 RIGHT 5 SOUTH LONGWALL BELT DRIVE AREA. MAN WAS HOLDING FIRE HOSE TO HOSE OUT UNDER DRIVE AREA. OTHER MAN TURNED ON THE WATER PRESSURE VALVE. PRESSURE CAUSED NUZZLE TO COME APART, END OF HOS E HIT MAN IN RIGHT JAW AREA.
PRESSURE HOSE USED TO SET DUKE JACKS AT HEADGATEWAS UNDER GOOSENECK OF HEADGATE ON THE BOTTOM. MAN PULLED ON THE HOSE. THE SHUTOFF VALVE ON THEEND CAUGHT ON SOMETHING & WAS INADVERTENTLY TURN ED ON. PRESSURE CAUSED HOSE TO WHIP & STRIKE MANON FOREHEAD CAUSING A LACERATION.
PREP PLANT OUTSIDE NEAR BUILDING. MAN HAD BENT DOWN BESIDE LOAD CENTER BUILDING AND WAS REACHING FOR AN IRON CHANNEL TO PUT IN END LOADER BUCKET. COPPERHEAD SNAKE BIT HIM THROUGH HIS PROTECTIV E GLOVE ON THE RIGHT LITTLE FINGER.
1 RIGHT 5 SOUTH LONGWALL FACE. MAN WAS BETWEEN #1 SHIELD AND MINE RIB, SHIELDMAN ON #2 SHIELD RAISE TOES OF #1 SHIELD. MAN SLID CRIB BLOCKS UNDER TOES OF #1 SHIELD TO RAISE TOES OFF THE BOTTOM . WHEN FINISHED HE STEPPED BACK AND RIB ROLLED OUT BETWEEN TWO DUKE JACKS AND HIT HIS LEFT KNEE.
1 RIGHT - 5 SOUTH LONGWALL HEADGATE. MAN WENT ON SOLID SIDE AND WAS PUTTING MONORAIL BEAMS ON HEADGATE BOX. AS HE UNCHAINED ONE END AND WAS MOVING IT OVER TO HEADGATE BOX, HE SLIPPED, CAUGHT H IMSELF WITH HANDS BUT MONORAIL BEAM CAME DOWN ON RIGHT HAND.
5 SOUTH SUBMAIN, CREW WAS TURNING CROSSCUT WITH CONTINUOUS MINER. WHEN GETTING PART WAY ROUND, THE NOTCH THAT WAS CUT WITH THE MINER WHEN TURNING THE ANGLE FELL OUT STRIKING THE VICTIM IN THE BACK AND LEFT LEG.
EE WAS CARRYING PARTS NEEDED FOR JOB DOWN STAIRS, FEET SLIPPED ON STEPS CAUSING EE TO FALL AND HE LANDED ON LEFT ELBOW.
1 RIGHT 5 SOUTH LONGWALL BELT ENTRY. TWO MEN USING 2 DRILL INSTALLING 12 FOOT CABLE BOLTS AS SUPPLEMENTARY BOLTS IN TOP OF BELT ENTRY IN ADVANCE OF LONGWALL FACE. WHEN ONE MAN WAS LOWERING DRILL TO PUT IN ANOTHER AUGER TO DRILL WITH, OTHER MAN HAD HIS HAND IN PINCH POINT BETWEEN JACK AND HANDLE.
SURFACE SUPPLY YARD UNROLLING A LARGE 12 FOOT ROLL OF 72 INCH CONVEYER BELTLINE, USING A CRANE TO HOLD REEL OF BELT UP IN THE AIR. SUPPLY CARS ARE MOVED BACK AND FORTH UNDER BELTING AND BELT I S LACED ON SUPPLY CARS. LARGE REEL SHIFTED AND CAUGHT MAN'S RIGHT LITTLE FINGER BETWEEN BELTLINEAND FRAME OF CRANE.
MAIN LINE TRACK TWO SET UP MEN WERE SWITCHING CARS AROUND. A SKID WAS USED TO SECURE 2 CARS IN ATRACK CHUTE. THE SKID KICKED OUT FROM THE WHEEL.VICTIM WAS REACHING FOR THE SKID WHEN FABRIC OF HIS PROTECTIVE GLOVE WAS CAUGHT BY THE FLANGE OFTHE WHEEL WHICH PULLED HIS HAND TOWARD WHEEL. THIS CAUSED HIS HAND TO BE CAUGHT BETWEEN THE WHEEL FLANGE AND THE MINE FLOOR.
MEN HAD ROTATED THE BOLT TO MIX THE RESIN AND HAD LEFT PRESSURE ON BOLT. THEY WERE UNTAGLING PRESSURE HOSE AND WATER HOSE WHEN DRILL PRESSURE BLED OFF AND DRILL FELL HITTING VICTIM'S LEFT HA ND.
EE STEPPED DOWN 2' OFF OF A CONCRETE PAD ONTO LOOSE COBBLESTONE & TWISTED HIS LEFT ANKLE. HE DIDNOT SEEK MEDICAL TREATMENT UNTIL 9/7/01. HE IS CURRENTLY WORKING RESTRICTED DUTY.
EE WAS UNLOADING GAS CYLINDERS FROM THE BACK OF A PICK-UP TRUCK. AFTERWARDS, HE COMPLAINED OF BACK PAIN.
EMPLOYEE OPENING 5 GAL BUCKET WITH UTILITY KNIFE. KNIFE SLIPPED CUTTING THUMB (LEFT).
SCREEN PLUG HIT HIM IN MOUTH AND BROKE TOOTH.
INJURED STEPPED INTO HOLE AS HE WAS WALKING THRU A PARKING LOT TO LOAD POLE AND SPRAINED HIS ANKLE.
LOCATION, IN A CROSSCUT ALONG THE MAIN LINE AT 14-07. MAN WAS PUTTING UP A PANEL FOR A KENNDY STOPPING. A SMALL THIN PIECE OF SLATE FELL FROM THE ROOF HITTING HIS RIGHT ARM BETWEEN THE CUFF OF HIS PROTECTIVE GLOVE AND HIS SHIRT SLEEVE.
WHILE EE WAS CUTTING BRUSH, DEBRIS (BRIAR) BLEW OVER THE TOP OF HIS SAFETY GLASSES AND INTO HIS LEFT EYE, CAUSING IRRITATION.
EMPLOYEE SLIPPED AND FELL ON FROZEN, SLIPPERY GROUND, LANDING ON HANDLE OF A CHAINSAW. THE IMPACT WITHT HE HANDLE OF THE SAW CAUSED A CONTUSION TO HIS RIB AREA.
