A roof fall was determined to be above anchorage at main line 15 belt, #26 crosscut. No injuries, travel ventilation are not affected. Dimensions are roughly 60' long x 16' wide x 12' high.
A roof fall above anchorage was found at main line 17 belt at #16 crosscut. No injuries, travel and ventilation are not effected. Dimensions are roughly 30' long x 12' high x 16' wide.
The blake ridge elevator stopped with 2 men aboard. the 2 men were removed from the elevator. Upon investigation a broken ""Governor safety latch catch arm"" was discovered. The entire governor wheel assembly was replace. the elevator was tested with MSHA & State inspectors before it was returned to service.
A roof fall above anchorage was discovered on main line #7 belt at #9 crosscut. The # fall is 55'long, 12'high, 16'wide no injuries occurred. Travel was effected ventilation was not effected.
A roof fall was discovered above the anchorage at 10-8 cc on the belt. No injuries; travel is effected; ventilation is not effected. The fall is approximately 70' long x 16' wide x 10' high.
A roof fall was discovered above the anchorage in 5 North #1 entry just outby #6 crosscut. No injuries. Ventilation and travel are not affected. Fall is 70' long x 16' wide x 12' high. Additional support has been set on both sided of the fall.
The Blake's Ridge Elevator stopped with 7 men aboard. We experienced an unplanned power outage which tripped a breaker on the BODE Air Compressor. The breaker was reset and the elevator ran normal.
A roof fall was discovered above the anchorage in the 17-06 crosscut belt to track. No injuries, ventilation and travel are not affected. The fall is approximately 20' long x 7' high x 16' wide.
6B Longwall encountered a gas well at #4 shield 30+30 spad. No injuries, no ignition.

A roof fall was discovered in the 5B tailgate in the crosscut at 27 wall at spad #27+80 Dimensions were 15' high x 15' wide x 20' long. No injuries. Did not affect passage or ventilation.
A roof fall occurred above the anchorage at 17-09 c.c. between the belt and track. No injuries. Not an escape way, travel and ventilation have been affected. Both sides of fall have been cribbed off. The fall is 40' in length x 15' high x 15' wide.
A roof fall above the anchorage at 13-20 in the Main East Return. The fall is in the c.c. 6-7 No injuries, no escape way travel and ventilation have not been effected. Breaker posts have been set on all approaches to the fall area.
Employee was raising the rib bolter and felt pain in his right groin. Employee was diagnosed with an inguinal hernia
Employee was assisting mechanics removing a spool pipe when the spool shifted and pinched his right hand. Employee fractured his right ring and middle fingers.
He was walking in the shower house on 11/3/12 and allegedly slipped twisting his right knee. He continued working with no complication until 1/8/13 when he began missing work.
Employee was at the 11A shear when a rock struck his right ankle resulting in a fracture. The shear was in the process of being serviced at the time of the accident.
Employee was pumping cement roof support cribs, on 09/29/2012 when a bag was filled it then created over spray and entered his right boot. Management was not aware of chemical burn until 10/01/2012 when employee reported. On day of occurrence employee worked his entire shift.
Employee stepped off motor in 12A section at 26 wall. He stepped on a rock and rolled his left ankle. The employee continued to work until 10/8/12, when he sought medical attention. An X-ray of his left ankle showed a fracture to his tibia. Employee did not begin work until 10/9/12.
Employee was struck in head with a piece of stone from the rib. Employee received a cut to his head requiring stitches and a hairline fracture to his C-2 vertebrae.
He was hand pulling cable slack when he felt pain in his left knee. *We dispute the allegations in Citation #8049155 and submit this report solely to terminate the citation.
EE was walking off belt line and stepped into a water hole and rolled his left ankle. EE went above ground at 8:00 am. He was taken by ambulance to Wheeling Hospital where he was checked out by a doctor and diagnosed with a high ankle sprain. Hospital gave him 3 days off/excused.
The employee was rib bolting on the 12A Section. A rock fell from the top, struck the continuous miner and then struck the employee's right foot. The rock fractured a bone in his right foot.
Employee was working with a cutting tool, cutting a brake shackle at the time of the incident. When cutting the brake shackle a piece of metal came back and struck Employee in the bottom lip.
He was lifting longwall flights when he felt pain in his lower back. *The operator disputes the allegations in citation #8055888 and submits this report solely to terminate the citation.
He was working on the mainline track. He was working off of a monorail car. He was leaning against the side of the car when he leaned back and his neck made contact with the energized trolley wire.
He was unloading bags of rock dust from a supply car on the 11 A miner Section. He was placing a bag onto the can duster when he stepped on a rock and turned his left ankle.

EE was carrying a bag of rock dust. He stepped on uneven ground and twisted his left knee. Results of MRI shows torn meniscus. Results came back to mine on 4/12/12.
Handling material in the 9A section. Material fell and struck individual in right knee causing pain. Results of MRI were received on 4/12/12. MRI showed a dislocated patella.
EE's left middle finger contacted the rotating gears in the loader duster blower assembly. EE fractured the tip of his finger and required sutures to close the laceration.
Tightening crib on inby side of can, when outby crib fell and struck right pinky finger.
EE was making a splice on a cable laying on the ground in 5 - 6 xcut on the 5N right side. The ram car operator slowly eased his ram car into the xcut unaware EE was there. He was struck by the ram car and was injured to multiple parts of body.
Employee was rib pinning on the right side of the miner in the section. He was drilling the first hole for his rib pin on the corner. The rib rolled out striking him on the right shoulder and behind the right ear.

Employee tightening rib bolt w/hydraulic drill. Drill spun & hit employee in right cheek.

A rock fell from the top striking his left hand. His hand was resting on the remote control while waiting on bolter to install bolts.
