A roof fall was discovered at 5 south #2 entry, # 6 cross cut. Fall measured 15' W. X 15' L. and was above the anchorage.

Initial report of individual trapped on grapevine elevator for 30 minutes prompted call to MSHA hotline. Upon resetting of the elevator by employee on surface it was revealed by the elevator time stamp that the individual was entrapped for 25 minutes which is not a reportable incident. No damage to elevator. Report being submitted to expire Hotline Call.

A roof fall was discovered @ 13-11 XC, #9 Entry of the Main East Return. Approximate dimensions of 20 X 20 X 12.

A roof fall occurred approximately 80' inby 14XC of 33 5 North Longwall. Approximate dimensions of 15' X 20' X 12'.

Struck by falling object

EE was drilling a left hole with pauline bolter, when drill steel got hung up, EE grabbed steels with left hand and checked bolter suction with right hand. The steel that was hung up in the hole fell, striking the steels he was holding, causing the drill steel to strike his right hand. Resulted in a fracture to the right ring finger.

EE was raising the head of the center bolter when a drill steel fell behind the head. The drill steel sprung back and struck the EE in the lower jaw. This resulted in a broken mandible and laceration.

Over-exertion in lifting objects

He picked up the ends of two ventilation tubes when he felt a strain in his back. *The operator disputes the allegations set forth in Citation #8051125 and submits this report solely to terminate the citation.

EE was assisting in supplying the miner in 5B. While carrying roof straps EE felt a sharp pain in his left shoulder after lowering arm. EE cont. working and did not miss work until 2/21/11. EE went off work for shoulder surgery on 2/21/11.

He struck his hard hat on the mine roof and felt pain in his head and neck. *The operator disputes the allegations set forth in Citation #8051123 and submits this report solely to terminate the citation.

Fall onto or against objects

He was walking along 10 belt when he tripped and fell striking the right side of his chest on a belt roller. *The operator disputes the allegations set forth in Citation #8045764 and submits this report solely to terminate the citation.

Over-exertion in lifting objects

Employee was at slope bottom lifting a safety rope onto car. Employee felt a pain on left side of lower back into the buttocks.

Caught in, under or between a moving and a stationary object

Employee was moving a cover for 7-A longwall headgate drive when another employee slipped resulting in injured EE pinching left hand in between the cover and the headgate drive. Original trip to hospital was diagnosed with a contusion of the left hand. Employee then went back to the hospital on 10-12-10 & was diagnosed with a fractured scaphoid bone in his hand.

Struck by falling object

Employee was operating loading machine on 9A section. A rock fell from the top, hit the loader canopy and struck employee's left middle finger, resulting in a fracture.

Over-exertion in lifting objects

Employee was lifting miner cable to place in cable hanger when he felt a pain in the left side of his groin. EE stayed on the job and came to work the next 6 scheduled shifts. Went to doctor on 10/7/10, diagnosed with hernia and began to miss work.

After carrying hoses along the 7 left canrow for approximately 8000 feet. Employee later complained of pain in left side of groin area. After receipt and review of medical records, it has been determined to be a reportable incident.

Employee bent over to pick up loose heel block when he felt pain in his lower back. After receipt and review of medical records, it has been determined to be a reportable incident.

Employee was unloading cribbing material from scoop bucket when he felt pain in right shoulder. Received medical report 9/1/10.

Employee was standing by cable reel while stretching out cable on the 4B longwall tail area. The handle on the cable reel was spinning and struck the employee on the right hand resulting in a laceration which required sutures

Over-exertion in pulling or pushing objects

While prying a piece of track rail up for bolt installation the ee experienced pain in his back. He returned to work his next scheduled shift but began missing work on 8/10/10. After receipt and review of medical records/excused on 8/11/10 this has been determined to be a reportable incident.

Fall onto or against objects

While working on the tailgate ranging arm a piece of material fell out of the face causing the employee to fall striking his forehead on the shearer resulting in a laceration requiring sutures.

Employee was tearing a wooden form apart and the pry bar slipped, causing the employee to twist his knee.

Absorption of radiations, caustics, toxic and noxious substances

Crew was spraying gunite on an overcast, when they had a plugged line. The crew located the plug, shut off the air to the hose and began to break the line. When the employee took the line apart, air and gunite shot out of the end strucking the injured in the face.

Fall to the walkway or working surface

While employee was carrying a sand prop he twisted his left knee, fell, struck his head and twisted his back.

EE slipped on a piece of rubber causing his lower left leg to rotate laterally causing pain in his knee.

Struck against stationary object

EE stepped on a piece of loose material causing his left ankle to turn causing a bad sprain.

Over-exertion in lifting objects

This 7000-1 is being submitted under protest in order to abate citation 8032396 issued on 11/4/10. The ee alleges an injury while on mine property while carrying K wall panels and experiencing back pain. As a result of a part 50 audit MSHA has deemed this a reportable injury.

Fall to the walkway or working surface

EE slipped on a miner cable while hanging a curtain, which caused him to fall on his left wrist.

Struck by falling object

While observing a shield being set on the 4B set up face, the employee started to stand up, pushing up with his right hand on the handrail, when a piece of stone fell out of the top smashing his right index finger resulting in an avulsion requiring sutures.

Employee was peeling a piece of rib with left hand when miner operator bumped the miner. His left index finger got caught between rib protector and rib.

This 7000-1 is being submitted under protest in order to abate citation 8032394 issued on 11/3/10. The EE alleges an injury while on mine property while stepping up into a motor and experiencing pain in his left knee. As a result of a Part 50 Audit, MSHA has deemed this a reportable injury.

On 1/18/10 ee experienced pain in his lower back while carrying ball buster jacks. EE returned to work his next scheduled shift but began missing work on 1/20/10.

He attempted to flip a flight on the face chain when he felt pain in his lower back. The operator disputes the allegations set forth in citation # 8051127 and submits this report solely to terminate the citation.

Struck by flying object

Employee was hitting two hammers together to cut a piece of screen, a splinter from the hammer struck his left leg. Employee received three stiches / no lost time.

He was dismounting the grader when he slipped and fell on his left side. *The operator disputes the allegations set forth in Citation #8051120 and submits this report solely to terminate the citation.

Employee was in the process of moving bolter cable. When he unlatched the hook, the latch sprung back striking the employee in the mouth. Resulted in chipped tooth.

Employee was setting a sand prop roof support in the 5 north right return when a rock fell from the top and struck him in the mouth.

Employee was running the headgate side of the shearer, when a baseball size rock came off the drum and struck him in the mouth.

While using a pry bar and hammer to straighten a bent motor step, the pry bar slipped and punctured his hand between the thumb and index finger.

EE unloading roll of belt on surface. While guiding crane, hook twisted, causing right hand to be caught between hook and metal frame encasing belt. Resulted in 7 sutures to right middle finger.

While standing in a cross cut to allow a shuttle car to pass by, the employee was struck below his lower lip by the passing shuttle car's trailing cable.

While closing the battery lid on the #60 scoop in 4BLW, the lid shut quickly pinching the EE's finger resulting in a fracture to the tip of his left thumb.

At 5 South 6 Right of McElroy Mine, bumped head on roof plank while crossing an old conveyor belt causing a piece of rock to fall and strike lower back.
