Struck against a moving object

Backed over bolder with loader and jarred his back.

Caught in, under or between a moving and a stationary object

While trying to realign the ram with the hole in the plate on the slide wrench for 013 drill the operator was holding the plate with his left and operating the ram with his right hand, The ram jerked forward pinching the tip of his finger between the plate and the slide wrench severing the tip of his left index finger.

A staging platform to open/close truck lids for loading cement powder had a step on the loading dock of the platform which needed resecuring. One step was lifted to expose the repair area of the step needing repair. While working, the lifted step fell and hit the left hand pinky finger of employee.

Over-exertion in lifting objects

Shoveling a pile of mud at counterweight of 2BC2 belt conveyer, and strained lower back

Fall to the walkway or working surface

Truck driver from customer (Double Eagle) fell on flat ground after checking his trailer while filling. Driver stooped under his trailer to check butter fly valve. when he turned to stand up he bumped his hard hat on a support beam and fell down breaking his lower right leg.
