Caught in, under or between a moving and a stationary object

While employee was removing a large piece of Rebond from Cyclone opening, EE pinched EE's finger between the rebond and the side of the opening. The result was a minor bruising of the finger.

Struck by falling object

3 workers were welding on a flat 5' X 5' steel door for a hammer mill (4BR1), the door was turned upright resting on the ground & leaned against a forklift for the final welding position. The door became unbalanced while the workers were welding, & fell over striking EE on the leg. EE was taken to the hospital by ambulance & was admitted for a broken leg.

Struck by falling object

Employee placed a metal rake against a post while waiting for a bucket to fill. Metal rake fell and hit employee in the back. Employee had a personal motorcycle accident 6/28/16.

Over-exertion in lifting objects

Employee was lifting a lock and load weighing about 20lbs. EE set the lock and load in the CAT bucket, stood up and felt a pinch in the back resulting in a strained back.

Employee was trying to loosen a hydraulic line on a unit and sprained lower back due to excess tightened fitting.

Struck by falling object

Employee was cleaning under the kiln feed and dust hopper when shove bumped the chute and material came out covering ee. Dust got in eye causing an irritation and burn to face and right eye
