Fall to lower level, (Not Elsewhere Classified)

Employee was inspecting walls between bag houses standing on electrical raceway, weld broke and he fell to deck below landing on feet. (8 feet) Not wearing Harness, nor following procedure of scaffolding the area. He received minor cuts and a sore back in the fall.

Fall from machine

(Weather - dry/calm) Accessed fall protection rack then activated air operated gangway to lower it to top of tank trailer. Operator then walked out across gangway to trailer to open lid. The gangway w/midrail started up on its own, exposing operator as he tried to grab it - lost balance and fell to ground.

Over-exertion in lifting objects

Employee was trying to turn a steel liner over and sprained his rt. torso

Employee was pushing an oversize tire over a gate as two coworkers were pulling on tire when it rolled backwards and tweaked his right wrist.
